Sub: Submission regarding name of witnesses in complaint of harrassment as required

in notice no. 1889/Doc/RFSL(Che.Puri) dated 27.09.2021.

Respected sir,
With reference to notice no.1889/Doc/RFSL (Ch.Puri) dated 27.09.2021, I am required to submit
the name of witness if any in support and process of my complaint.
I most humbly submit that my complaint contains multiple issues & incidences and there are
several witnesses accordingly. I had already requested to you for providing me a letter with
specific queries mentioning the details/of incidence and name of witness required on separate
issues, as mentioned in complaint. However, to best of my knowledge and memory I had
prepared a list of witnesses for separate issues/incidence etc. in my complaint as below:

1. Regarding irregular functioning of stoelting Polygraph system:

(I). Ms. Deepa Verma, DFSL: To whom I informed in writing along with enclosures of the graphs
that stoelting Polygraph System was and is not working properly;
(II). Ms. Asha Pahwa, TM/FPD: To whom I informed in writing along with enclosures of the
graphs that stoelting Polygraph System was and is not working properly;
(III). Sh. Aman Kumar Yadav, Purchase officer/SSO(FPD):To whom I informed in writing that
stoelting Polygraph System was and is not working properly;
(IV). Sh. K.C. Varshney, QM FSL: To whom I informed in writing that stoelting Polygraph
System was and is not working properly;
(V). Sh. Shrinarayan, HOO FSL: To whom I informed in writing that stoelting Polygraph System
was and is not working properly;
(VI). Ms. Kavita Goyal, Calibration officer:To whom I informed that stoelting Polygraph System is
not working properly;
(VII). Mr. Gautam Thawani, Service provider of Stoelting Polygraph system: Who visited the
laboratory on complaint of irregular polygraph system and who confirmed that if real recording
is not the same as the printed graphs then the system is malfunctioning.
He was also informed about the theft incident on his visit to the lab for installation of the
system, equipment accessories & goods.
(VIII). Two other officers in the division namely Dr. Aruna Mishra AD(FPD) & Dr. Himakshi
Bhardwaj AD(FPD),also to whom I submitted the same observations.

(2). Regarding my delayed promotion & lost APAR matter:

(I) Sh. Herman khujur, SO FSL: who informed me that 'the promotion file of two SSA's had
been sent for DPC for recommendations of promotion but your file is left in the office and your
APAR is missing from it."
(II). Sh. Shrinarayan, HOO FSL: to whom I informed in writing about my file being left out for
promotion, missing of APAR & representations for rectification of APAR.
(III)Ms. Deepa Verma DFSL: To whom I requested regarding missing APAR, forwarding of the
promotion file & rectification in the APAR.
(IV). Ms. Asha Pahwa, TM FPD: to whom i requested to APAR reporting in time for my due
promotion.

3. Regarding the key's of room no. 21 the seating & record room which was taken forcibly by
Sh. Puneet Puri AD(Ballistics)/ HOD(FPD) on 29.05.2020 without official handing/taking over &
had put up one more lock upon my locked cabin:

(I). Ms. Deepa Verma DFSL: to whom I requested several times verbally to resolve the matter
and had also submitted written representations for its timely disposal.
(II). Sh. Shrinarayan, HOO FSL:
to whom I requested verbally several times to resolve the matter and submitted the
representations for it.
(III). Sh. Puneet Puri AD(Ballistics) HOD(FPD): to whom I requested verbally several times to
resolve the matter and submitted the representations in this regard;
(IV). Sh. Saini, Security Officer FSL: All representations were copied to him on this issue .

Two other officers namely Dr. Aruna Mishra AD/FPD & Dr. Himakshi Bhardwaj AD/FPD who
were also sharing the room no. 21 at that time with me.

The undersigned had put up my least efforts to prepare the list of witness including the
reference of incidence and issues.
