    Sub: Request to expedite the disposal of my all representations, file notes, replies of all
    memorandums/advisory for producing the evidence to the Inquiry Committee in reference to the
    notice no. 1889/Doc/RFSL (Ch.Puri) dated 27.09.2021.

    Respected Ma'am,

    With reference to notice no. 1889/Doc/RFSL(Ch.Puri) dated 27.09.2021. I am directed by the


    Chairman/Inquiry Committee to be present before the Inquiry Committee on 01.10.2021 along
    with my all evidences and relied upon documents accordingly.
    I wish to draw your kind attention to the list of following long pending matters which are not
    disposed by your good office till date:
    1. File notes dated 19.11.2018 regarding Stoelting Polygraph system;

    2. Relied upon documents, noting of order dated 28.11.2018 regarding withdrawal of


    reporting from me;

    3. Representations dated…. regarding withheld of promotion file, removal/ missing of


    APAR from the records, complaint against poor grading & rectification in APAR;

    4. Representations dated…. regarding access of keys, Almiraha, table drawers, all records
    kept in room no 21 since 29.05.2020 onwards and video graphy of incidence regarding
    access of records, CCTV footage & entry details of committee members & other officers
    involved this matter;

    5. Representations dated… regarding inspection of relied upon documents of order

    6. Representations dated… regarding for providing the cabin & Almiraha allotment order;
    7. Representations dated…. regarding reply of all memorandums/advisory;
    8. Representation dated…..for the list of official email and mobile number of the staff and
    officers of ld fpd

    Madam, since this inquiry Committee is being constituted upon your direction for the redressal
    of my complaint regarding harrassment at work place.
    All relevant documents and evidences are also to be obtained by me from your good office and
    HOO/HODFPD You are requested kindly instruct to them for expediting and cooperating for the
    same.
    Madame since the committee is constituted by your orders i humbly request you to help me
    getting justice from the positive outcome of it and not letting it conclude in hasteness without
    seeing corrobarative evidences and giving me the chance to collect them.
    It would be violation of principle of natural justice and my fundamental rights if it concludes
    without hearing to my side of story and truth.
    I will be deeply obliged on your this act of kindness.
