CONTRACT OF EMPLOYMENT

KNOW ALL MEN BY THERE PRESENTS:

This Contract made and entered into by and between:

JAKE ELTON C. SANTUYA, of legal age, Filipino, married

and a resident of P-1, Lianga, Surigao del Sur, hereinafter
referred to as the “EMPLOYER” ;

And

KEN ELCID C. SANTUYA, of legal age, Filipino, married

and a resident of Lianga, Surigao del Sur, hereinafter
referred to as the “EMPLOYEE”

-WITNESSETH, that:

WHEREAS, the EMPLOYER is the owner of a parcel of the

PHILIPPINE DRUG SCREENING LABORATORY, with office and
business address at Rm. #2, Capitol Road, Telaje, Tandag
City, Philippines ;

WHEREAS, the EMPLOYEE is an Authorized Specimen

Collector (ASC) and employed as such position per description
of DOH Manual on Operation of Drug Screening Laboratory ;

1. That the said employee is an Authorized Specimen

Collector (ASC) is employed on full time basis at
Philippine Drug Screening Laboratory with office
hours 8:00 AM to 12 noon and 1:10 PM to 5:00 PM ;

2. That the employee in case of reservation shall

endeavor to come by applicants to be drug tested on
rest day and shall bind himself perform his
function to service that said applicant during off-
days when applicants who have previous appointment
with the head of the laboratory with the consent of
Analyst in charged ;

3. That the salary is P5,000.00 per month ;

4. That the seminars/trainings in relation to drug

testing is shouldered by the employer.

IN WITNESS WHEREOF, the premises hereto have hereunto

Francisco, Agusan del Sur.
