are under a French contract of employment?

What is the French Social Security?
As a resident of France, you must get a new domestic insurance based in France. Therefore, you will no longer be
attached to your country of origin and you will contribute to the French regime (URSSAF). For children, please go
to appendices 2.

What documents will you need to register yourself to Social Security?

1. You are either from E.U or EEE - European Economic Area (Island, Liechtenstein, Norway)

-Fill in the form S1106 " Demande d'ouverture des droits à l'assurance maladie" (available in appendices 1). You
can also download it on https://www.ameli.fr/sites/default/files/formulaires/168/s1106_-_demande_puma_-
_assurance_maladie.pdf
-Copy of the passport;
-Copy of the birth certificate with filiation in the first language and in French (mandatory), prepared by the
embassy or consulate or by a sworn translator on the list of experts established by the Courts of Appeal and the
French Court of Cassation (www.courdecassation.fr).
-Copy of the valid residence permit;
-Bank details;
-Copy of your first pay slip;
-Copy of a proof of residence less than 3 months old.

2. You are from another area

-Fill in the form S1106 " Demande d'ouverture des droits à l'assurance maladie" (available in appendices 1). You

-Copy of the passport;

To whom do you send all these documents?

Health insurance fund (called CPAM in France) based on your area of residence (available in appendices 5).

What is the Carte Vitale?

Smart card which contains your administrative information required to provide you with medical care:

▪ Your identity and that of your beneficiaries


▪ Your social security numbers (15 figures: x xx xx xx xxx xxx xx)
▪ Your health insurance scheme (“régime general”)
▪ Your CPAM in accordance to your dwelling place

Gender (“1” for Man, “2” for Woman)


Year of birth
Birth month
Place of birth (99 for foreign person)
Area code (according to INSEE)

The Carte Vitale is issued free of charge by you Assurance Maladie. If somebody asks you for money, it is a fraud

What is the Vitale Card for?


It allows a healthcare professional to draw up electronic treatment sheets instead of paper treatment sheets, and
to transmit them electronically to your health insurance fund (CPAM). It therefore simplifies the administrative
relationship between you, the healthcare professional, Health Insurance and Harmonie Mutuelle, and allows you
to be reimbursed faster.

How to get your Carte Vitale?

1) Go on Ameli.fr (link below) and create your own account

Ameli account is the service for all people registered to Régime Générale (this will be your case). It is your secured
personal space for you to access all services from your computer, mobile or tablet. You can check your
reimbursements, download your certificates, get your Health Insurance European card (CEAM, in French), contact
an advisor by email, etc.

You will only be able to do this once you have received your definitive social security number (you cannot do it
with your temporary number).

How long is the Carte Vitale valid for?

The Carte Vitale does not have deadline. If you have specific rights (additional CMU, exemption from user fees,
etc.), they are registered on your Carte Vitale as well as their period of validity.

Your Carte Vitale may be requested by

What should I do if my situation changes?

Notify your health insurance fund (CPAM) on your Ameli account or by letter to your own CPAM (marriage, birth,
removal, etc.) and go to a pharmacy or into your CPAM to update your data on your Carte Vitale.

How can I do to get reimburse of all my health expenses without my Carte Vitale?

If you do not have your Carte Vitale or you have not received it yet, keep all your documents (i.e: care sheets,
bills, etc.) in order to send them both to your CPAM and Harmonie Mutuelle once you are temporarily registered.

Note that you will be able to make claims from the date start of your contract of employment from both CPAM
and Harmonie Mutuelle.
i.e: Today it is May 29, 2020 and you have just got your Carte Vitale, but you were hired on January 6, 2020 in
France. Over that period, you have had 5 care sheets, several orders of medicines and a new pair of glasses. After
having your temporary number, you will at last send all your documents to both your CPAM and Harmonie
Mutuelle.

How to get a General Practitioner – GP?

Prior to getting a GP, you must activate your Carte Vitale in a Pharmacy.
Declaring your choice of GP can be made online by the GP, with your approval during a consultation at its office
and on presentation of your Carte Vitale. The doctor you have chosen as your GP makes the declaration online
and transmits it directly to your health insurance fund. It’s quick and easy: no form to fill out, no mail to send and
your declaration is recorded immediately by your health insurance fund.

What is Harmonie Mutuelle (HM)?


It is a private complementary health insurance which is mandatory at Total once you are hired in France. It covers
costs such as drugs, hospitalization, care, that CPAM does not support. So, you are able to be reimbursed for more
thanks to this private insurance. You can be reimbursed by HM if you have received your final number (social
security). You must send to HM your bank details in order to get your refunds.

Your membership is created by your CORPER (advisor in all human resources administrative tasks) but you
absolutely have to add your personal data on your own account regarding your family status and your
beneficiaries. Do not hesitate to contact your CORPER for any support.

You have 2 options:


 If you have a family (children and/or spouse) ➔ you must take option called “Famille” A line on your pay
slip will be clearly
 If you are single and/or without children ➔ you must take option called “Isolé”.
