ACTION

-I was able to assess the family using the family assessment tool in order to uncover some discrepancies in the household and be able to report
my findings to my Leader. I was also able to provide health education to my clients after uncovering the discrepancies after I have done assessing
them.

Reflection

- It gave me a lot of new insights on how to assess the family using the family assessment tool in order for me to uncover some
discrepancies andto provide Health education to the family.

Faith

-For with God, nothing is impossible- Luke 1:37

-Before I started doing these activities I thought to myself that this is impossible due to my nervousness. So I prayed to God so that he may be
able to guide me in doing this community health service to the families in the barangay.

Action

-First is that I will always pray before dping these activities so that it will boost my confidence and so that God may be able to guide me in doing
these activities.
