Description of Incident:
Diesel fuel leaked from Tank 1 around 10 P.M, Tuesday night, of May 16, 2017 and
flowed into the storm drainage canal going into _______Creek leading to ____River
causing oil spills in the creek prior to discovery on May 17, 2017 at 7:00 AM
Estimated Amount of Chemicals, Hazardous Wastes, Emissions, and/or Effluents
Released to the Receiving Environment:
An estimated 10 drums of Diesel fuel flowed out of the plant premises.
Probable cause of the Incident:
Diesel fuel leaked out of the storage tank due to a damaged gate valve. The strong
rain in the night of the incident swept the fuel from the plant storm drainage canal
into the nearby creek which leads to ________River.
Interim/Contingency Measures to Mitigate any Potential Negative Impact:
PCO informed CDC and placed spill booms in the creek to contain the oil spill. Clean up
recovery was done by PCO employees & witnessed by CDC EPD. All the oil spills,
sludge, boom were collected and recovered from the nearby creek for future
treatment and disposal by DENR accredited treater. 15 drums hazwastes were
collected.
