I would like to request from your good office to allow us to use the Sales Invoices and Collection
Receipts of our head office for our new store located at Space 3-021 3F Greenbelt 5 Ayala Center Legaspi
Street San Lorenzo Makati City.

We are encountering a delay in processing of the new Authority to Print due to the current working set
up and shortage of manpower caused by Pandemic.

We have provided the details below. This will be used for an estimated period of 1 month from February
1, 2022 to March 1, 2022 while wailing for the processing & printing of Store’s sales Invoices &
Collection Receipts.

Collection Receipts: 81551-81600

We attached the following documents for you reference:

 Payment Form ( BIR 0605) of Head Office and Store

 Photocopy of COR of Head Office and Store

 ATP Application of head office

 SI and CR  borrowed from Head office with the Stamp address of the store
