4. OOA 1000H OJT Antonio instructed by NUP Pascual to interview the subject to investigation.
5. OOA 1100H Team Charlie took their lunch.
6. OOA 1130H in relation to number 5 Team Charlie had returned to Balagtas MPS.
7. OOA 1130H Team Delta took their lunch.
8. OOA 1200H in relation to number 7 all of Team Delta had returned to Balagtas MPS.
9. OOA 1300H OJT Nicolas, Asanza and Cruz instructed by NUP Pascual to took up fingerprints from
the detaiiness.
10. OOA 1700H PO2 Paulino M Iglesias gave us instructions that were having our first quiz, and he
also orien us on the rules and regulation and the other matter about Balagtas MPS.

2. OOA 0730H PO2 Iglesias instructed us our designation and tasking for our duty.
3. OOA 0800H OJT students move to their respective designation and wait for further instructions.
4. OOA 1100H Team Delta took their lunch.
5. OOA 1130H Team Delta returned to Balagtas MPS.
6. OOA 1130H Team Charlie took their lunch.
7. OOA 1200H Team Charlie returned to Balagtas MPS.
8. OOA 1300 PO2 Iglesias make a phone call to inform OJT Bondoc to deplot two (2) OJT students
known as OJT Del Rosario and OJT Espejo at Barangay Borol 1 st to make an area security for the
visitation of Vice Governor Daniel Fernandoto the Medical Mission.
9. OOA 1330H OJT Del Rosario and OJT Espejo went to barangay borol 1 st in relation to number 8.
10. OOA 1400H OJT Peyra and OJT Estrella instructed by PO2 Iglesias to help him to the paper
works.
11. OOA 1650H in relation to number 8 they’re both returned to Balagtas MPS.
12. OOA 1658H OJT Espejo took an early out for for personal reason.

5. OJT Del Rosario and Peyra instructed by PO2 Iglesias to build a stand for the tarpaulin.

Malolos Provincial Hospital.
