This is to certify that based on the inspection report of the Cooperative’s Audit
Committee dated November 23, 2020 the fifteen (15) units Class 2 modernized jeepneys
delivered by the DURABUILT MOTORS, INC. (Supplier), are free from defects, in good
running condition, complete with accessories and in accordance with the specifications as
quoted by the Supplier.

It is further certified that the fifteen (15) units Class 2 PUJs more particularly
described below were accepted by Cordillera Basic Sectors Multipurpose Cooperative on
November 24, 2020.
