Instructions to Faculty:

 No synchronous and asynchronous class sessions shall be held during the


examination period.

 Faculty should remind students to check their Departmental Examination


canvas course card weeks before the date of examination.

Instructions to Students:
 The examinee shall be responsible for securing a gadget and stable internet
connection during the conduct of the examinations.

 The examinees are discouraged from taking the examination in a computer


shop or any public place to avoid health risks. Inter-Agency Task Force’s
health protocols must be strictly observed.

 Each student shall be given sufficient time to complete the examination in


accordance with the approved schedule.

 The student shall not access any resources during the exam, including Canvas
Modules and Canvas Module resources, external websites, other software
applications, or any hard-copy resources. All other web browser tabs must be
closed during the exam. Mobile phones and all other electronic devices must be
turned off for the exam.

 Students must follow the specific instructions indicated in the guidelines given
per examination.

 In case the student fails to take the online examination, the decision about a
special examination shall be at the discretion of the course committee, subject
to submission of letter of explanation for the failure to take the examination
during the regular schedule. Pertinent documents and evidence must be
attached.

In case of problems that may arise during the examination proper (i.e., like
failure to submit the answer sheet in Canvas due to the internet disconnection),
the student should report the incident as soon as possible to their professors or
instructors through e-mail or other available communication channel. Pertinent
documents and evidence must be attached.

Letter of explanation will be deliberated by the exam committee and the


department. Once approved, a special exam shall be prepared and concerned
student shall be notified for the schedule. Corresponding fees may apply.

Policy on Special Exam (Student Handbook):


A. Students who missed the regular examinations need to apply for Special Examinations
directly at their respective Institute by filling out a Special Examination Form.
Application period covers three (3) school days immediately after the last day of the
regular examination schedule.
B. Students have to pay directly to the Cashier. The Cashier will issue an official receipt
for Special Examination Fee.
C. Students then proceed to the Institute for proper scheduling of special examination.
D. Students should claim their grades from their respective Institutes.

 Students are expected to always act as mature individuals. They are expected to
observe honestly before, during, and after the online departmental examination.

 Provisions in the Student Handbook and other university policies governing the
students will remain in effect unless suspended by the Academic Affairs Office
(AAO) or by the University President
