Guide on Reporting of COVID-19 Cases

Description Confirmed Cases Staff who were Close Contact to A

You are considered as a Close Contact if you have interacted with a positive
case within 1 meter and were in any of the following situations:

i) More than 15 minutes

ii) Interacting without a face mask
iii) Physical contact
iv) Identified by MySejahtera as a Close Contact
Proposal for
(a) Treatment of working day Hospitalisation leave based on documentation provided by KKM 1) Do COVID-19 test
2) Pending test results - Work from Home or Unrecorded Leave
3) If results negative - to complete quarantine period as advised by KKM:
Work from Home or Unrecorded Leave
4) If results positive - follow Category A
5) Update MySejahtera

(b) Medical cost Borne by KKM/Borne by UC/Borne by staff Borne by KKM/Borne by UC/Borne by staff

Document Required Documentary evidences. Eg. Letter from KKM/COVID-19 test results/COVID-19 Documentary evidences. Eg. Letter from KKM/COVID-19 test results/COVID-19
test receipt/MySejahtera Screenshot etc. test receipt/MySejahtera Screenshot etc.

Note : (1) It is the responsibility of the staff to notify his Dean/Head immediately if they are tested COVID-19 positive or close contact.
Thereafter, Dean/Head to notify Crisis Management Committee.
(2) This guide may be updated in response to the COVID-19 situation which may be volatile from time to time.
(3) All Fac/Cen/Dept/Brh and staff are reminded to check this guide in the Staff Intranet from time to time.
(4) The instruction to Work from Home or be granted Unrecorded Leave shall be at the discretion of the Dean/Head.
