Dear Teacher,
In order to make our cheer squad or be a mascot we require accountability for their
behavior in class. We would ask that you help us gather some information about one of
your students that is applying for the 2011-2012 season.
Please rate the student in the categories below and return to the student. If you would
like to make this confidential, you can fill it out in private and respond to me directly or
leave it in my box in the mail room. Thank you for your cooperation, and feel free to
contact me if you have any concerns. Feel free to write any comments on the back of
this evaluation!

Thank you for your interest in the Evergreen Valley High School Cheerleading
program. We are excited about upcoming tryouts for the 2011-2012 school year.
Attached you will find all forms needed for tryouts. A current physical is needed to try
out.
Tryouts have been scheduled for April 11 th- 15th. We will not accept late applications.
If you do not turn in your application by the deadline (April 8 th), you will not be able to
participate in tryouts.
These will be CLOSED practices. We will not allow any spectators. By April 8 th, you
must have all required elements:
1.) Application
If you do not have all items turned in you will not be permitted to tryout.

Cheerleading/Mascot Contract
Cheer is one of the most demanding sports that a high school student can undertake.

It demands not only physical abilities, but also requires much of the student's time and
commitment. In addition, it is a position demanding leadership skills and participation in
school service related activities. I, ________________________________, realize that
being a member of the Evergreen Valley High School Cheer program is a privilege and
honor. I accept responsibility to my school and squad if I make the team. I also
recognize that my first commitment to Evergreen Valley High School and myself is my
education. As a member, I am going to abide by this contract and be a great role model.
Eligibility
Each member must maintain a 2.0 GPA or above. One must meet the application
deadline. A candidate may be male or female. One cannot owe money from previous
seasons. If you quit the squad one must wait for a full season to pass to try out again
the following year. Example: If you quit when you were a freshman then you cannot try
out again until your junior year. Acceptance of a candidate shall be on basis of ability
and acceptance of squad/school rules.
Cheer/Mascot Camp
All Cheer team members are required to attend camp, scheduled for July 29- August 1 st,
2011 at UC Santa Cruz University. Fees must be paid to “EVHS-ABC” cheer account by
the cheerleader/mascot. The cost is $367.00 which includes meals, instruction and
rooms. Transportation is not included but we will arrange carpooling. The registration
fee deposit of $25.00 and the 2nd deposit of $25.00 by May 5th will offset this balance to
total $317.00. Fundraisers and sponsorships will be offered to offset the cost and make
the program available for anyone who works hard and shows us their commitment.
Coaching Fee

Coaching fees will be $20.00 a month per cheerleader. The fee will be due by the
5th of each month. The first payment will be due by May 5th for the month of May.
Competitions
Our goal this year is to participate in at least one California competition, however, it
depends on your commitment as a team member. We will do our best to get you ready.
We will also fundraise to try and offset the costs. Please note these fees are determined
by the facility that holds the competitions.
Attendance: Cheering at Football, Basketball, and Playoff Games, Parades, etc: Cheer
is a full year commitment from May to March. Every student will be required to attend
every cheer event and be represented at most sports that go to CCS. Confirmed dates
for CCS games are difficult to determine in advance, however attendance is mandatory.
You must wear your uniform to school on a game day. Please do not try to get creative
with your uniform. Your uniform is your shell, liner, skirt, tennis shoes and warm ups.
Each squad will have the same attire and hairstyle to show unity. To perform at half time
you must be at practice the day before. All cheerleaders/mascot must be at the game
thirty (30) minutes before the game starts unless instructed differently by your Advisor
and Coach. Attendance will be taken by the Coach/ Advisor at every game. Late arrivals
will be considered tardy and will result in you receiving demerits.
Cheerleaders/mascots are required to remain for the entire game unless
instructed otherwise. Leaving early from a game will result in receiving a demerit.
Performance Rules
If you are not cheering you need to be sitting with your squad in the bleachers
supporting your team. You may not sit with mom, dad, sister, brother, friend and/or
boyfriend/girlfriend. If you are performing stunts you will remain focused. The squad
who is performing is in charge of calling the cheers. The squad not performing is
supporting them. Captains and co-captains cannot have their cheer list during the
games. During football season you are in formation ready to cheer for the remaining two
minutes of half-time. During basketball season if you perform during half-time then you
are in formation ready to cheer for the remaining four minutes of the 3 rd quarter. If you
do not perform you are in formation ready to cheer for the last two minutes of half-time.
Each cheerleader must understand that games and performances are not opportunities
to learn cheers. Every cheerleader in uniform is responsible for knowing each and every
cheer that is called throughout the course of a game. Any cheerleader who is learning
or trying to work through a cheer while in the performance area will receive a demerit.
If you do not know the cheer please stand attentively with arms behind your back and
wait for the next cheer. Please understand that practice is the time to learn, make
mistakes and ask questions to your captains and coach.

Dress
Proper cheer uniforms will be required at all school functions, on game days and
during school hours. Cheerleaders must follow the "what to wear" list determined
by advisor/coaches. Each member will be required to maintain good
condition/cleanliness of his/her uniform on a weekly basis. Cheerleaders will arrive at
games already suited up and ready to cheer. There will be no changing into uniforms,
removal of jewelry at games. Be prepared and ready to go! The wearing of an
incomplete uniform (different shirt, wrong shoes, jewelry, no kick pants etc.) will result in
demerits. The Mascot however will be allowed to change at games due to the uniform
outfit. Cheerleaders in uniform or wearing any item of squad or team identification must
always conduct themselves professionally.
Appearance
All hairstyles must be neat and secured up out of face, and not needing attention while
performing. Nails must be kept at proper length, short and no fake nails. No jewelry
worn during cheer time. Makeup should be natural and not excessive. Cheerleaders
should look their best at all times, especially when in uniform. Inappropriate appearance
will result in demerits.
Conduct
During the season squad members are required to maintain and uphold the reputation
of cheerleaders though their own conduct and squad spirit. Set a good example at all
times that could not be criticized. There are no excuses for disorderly conduct (smoking,
drug/alcohol use, fighting, cursing, and disrespect to faculty and adults). Cheerleaders
and mascots caught participating in any of these behaviors will be dismissed from the
squad and discipline will be handled by the school's administration. Sportsmanship will
be demanded. All cheers /chants shall be of a positive manner. Members will make
every attempt to ignore or dissuade negative responses at events and not become
involved in such action themselves. All attitudes must remain positive and constructive;
consistent failure to do so may result in dismissal from squad. If you have an issue that
you feel you cannot handle professionally with another person please contact your
coach or cheer advisor. All personal items must be put away during games. All
cheerleaders must be prepared to cheer and perform during the game, time-outs and
half times.
While in uniform, no public displays of affection (hugging and kissing) are allowed. You
are representing the school and should hold yourself to high standards. During games
there will be no socializing with athletes. They have a job to do and so do you. While
cheering there will be no socializing with fans. There is no socializing with
boyfriends/girlfriends during practice. Inappropriate conduct will result in demerits.

Any behavior that is a poor reflection of the school and your squad, whether on
campus or off, will be cause for dismissal from the squad at the discretion of the
Coach/Cheer Advisor.
Practice
We realize that the time requirements (including summer) may interrupt traditional
work hours and family vacations or plans. However, once you receive your schedule
please plan accordingly. Cheerleaders/mascot are required to wear suitable workout
clothing and shoes (no jeans, sandals, jewelry and/or Uggs.) No Gum Chewing. These
rules are to keep you and your team safe. You need to be dressed and ready to practice
at designated time. Attendance is mandatory at every practice. If a student has a non-
cleared absence and fails to clear it, it will be counted as a demerit and dismissal from
squad will be considered. If a cheerleader/mascot is to miss practice due to illness or
injury, the Coach should be notified prior to the practice and a note is required upon
your return. If it is an injury a note it required from your physician. All Cheerleaders will
arrive and leave on time. Arriving late and leaving early to practice will result in
demerits.
Grades
All cheerleaders/mascot are required to maintain a 2.0 or above GPA. If you take AP
classes your GPA maybe evaluated individually. If you become ineligible for two grading
periods, you will be removed from the squad, forfeiting all monies and time invested.
ASB Card
All cheerleaders/mascot are required to get a physical. This form must be completed
and turned in prior to trying out. If you played another sport, your physical is good until
school starts. Please make sure that we have a copy of the physical for our records.
All Cheerleaders/Mascot will be required to have a new physical for the upcoming
school year.

Fundraising
Our first fund-raiser details are being worked out. Students are expected to
participate in all fundraising activities. Certain fundraisers will help contribute to
the individual cheerleader for competitions.
Honor System
There is a difference between snitching and holding your team accountable. I expect the
team to be held accountable.
Demerit System
A. Points will be used as a disciplinary tool.
B. The Cheer Advisor will give them.
C. Anytime you get a point you owe me a 1 –mile around the track.
D. Three points will result in the removal from a performance
E. Six points will result in suspension for two weeks
F. Twelve points will result in dismissal from team.
G. One point will be given for:
1. Tardiness at any team activity
2. Chewing gum with uniform
3. Chewing gum at practice
4. Uniform inspection
5. Disrespect toward coach or team
6. Inappropriate practice/performance uniform
7. Forgotten props and accessories
8. Inappropriate performance make-up
9. Practicing cheer during games
10. Late to your formation
H. Three points will be given for:
1. Unexcused absence

2. Use of inappropriate language in uniform
3. Gossiping in uniform
4. Bad sportsmanship
5. Missing a performance for the first time
Second time=four points
Third time= five points
Total of three performances missed will result in dismissal
I. Six points and suspension, will be issued for violations of above infractions
totaling 6 points or:
1. Below 2.0 GPA
2. Talking to athletes during a game
3. Displays of affection while in uniform ( hugging and kissing
boyfriend/girlfriend)
4. Hanging out with boyfriend/girlfriend during practice
J. Twelve points, or dismissal, will be issued for violation of above infractions
totaling 12 points or:
1. Disorderly conduct (smoking, drug/alcohol use, fighting and disrespect
to faculty and adults)
2. Second time below 2.0 GPA
K. Suspension= not performing for two weeks but supporting your team in warm
ups.
I have read and understand the significance of this contract. I understand all of
the responsibilities of an Evergreen Valley Cheer Squad Member. I am keeping a
copy of this contract for my records in my cheer folder.
