INSTRUCTIONAL SUPERVISION – MONITORING TOOL
MODULAR DISTANCE LEARNING MODALITY

2. The learning activities are aligned to the target learning competency for
the week and quarter when the monitoring was done.
3. The learning activities to be done by learners are reflected in the Weekly
Home Learning Plan (WHLP) of the learners and/or Lesson Exemplar (LE)
of the teachers.
4. Learning activities are organized in such a way that learners can perform
the different tasks from the most basic with increasing complexity.
5. The level of difficulty of the learning activities in the module / activity
sheets / worksheets are within the range of ability of the learners
according to their grade level.
6. Each learning activity contains clear directions on what to do and how it
will be graded (if performance task) and/or relevant examples as guide for
the learners.
7. The learning activities are varied and addresses the learners’ diversity.
2. MDL SUPERVISION
1. The teachers have a prepared WHLP for learners which are consistently
checked by the school head and/or subject head.
2. The teachers are able to monitor the compliance of learners to the
requirements within the specified timeframe and remind those who are
falling behind schedule.
3. The teachers have a created a mechanism / system to provide timely and
appropriate feedback to the learners.
4. The teachers are available for consultation and is able to frequently
answer questions from learners and parents.
5. The teachers use assessment/s that is/are consistent with curriculum
requirements / standards.
6. The teachers have a clear record of the learners’ output and is able to
track their progress / development.

C. ISSUES / CONCERNS
1. Rate the issues / concerns experienced by the learners, parents / guardians, and
teachers during the conduct of modular distance learning using the guide below.

1. There are significant grammatical and/or typographical errors
found in the module and/or worksheets / activity sheets.
2. There are significant errors in the content of the module and/or
exercises in the worksheets / activity sheets.
3. The explanation or discussion of the topic in the module is
inadequate.
4. There are blurred images, missing pages, and unclear print in
modules and/or worksheets / activity sheets.
5. There are incomplete or no answers at all in the modules and
worksheets / activity sheets that were returned.
6. The learner WAS NOT the one who answered modules and
worksheets / activity sheets.
7. There is no learned adult at home who supervises the learner at
home when studying and answering the learning materials.
8. Modules / activity sheets / worksheets were not retrieved and/or
returned on time or not picked-up and/or returned at all.
