VIDEO AND INTERVIEW CONSENT AND RELEASE FORM

Before agreeing to participate in this documentary film, it is important that you read and
understand the following explanation of the proposed procedures. It describes the description,
procedures, benefits, risks, and discomforts of the documentary. It also describes your right to
withdraw from the study at any time.

Description of the Project

Twitter is a social media app that is used for various things, but little do some people
know that within the app, comfort, pleasure, secret transactions, and survival is a part of a
secret world, the ALTER world. The documentary aims to open people's minds to the fact that
online sexual engagements exist, and many young people today are involved. The group will
look for Gen Zs who are willing to share their stories regarding how and what their lives are like
when they mask up for their “alter accounts” and what it is like once that mask comes off. This
documentary will make you want to ask the question: “Should we judge these people that are
involved in this set-up?” or understand and know more on why they really are a part of the
alter-world?

The Procedure of the Interview:

● The interview will be conducted via Zoom.
● Before the formal interview begins, creators will begin recording the online meeting or
call via Zoom.
● All journalists of the team are present during the interview process. However, one host
will solely ask the question to the subject scheduled for the interview.

Potential Benefits:
By participating in this documentary, you will get an opportunity to provide information
about your experiences with the alter-world, identify positive and negative aspects of the
alter-world; and potentially raise awareness about the alter-world on Twitter. Your responses are
highly valued and could possibly explain the alter-world to the Filipino mass and de-stigmatize
the misinformation and disinformation surrounding this community.

“Maskara” the right and permission to copyright and/or publish, reproduce, or otherwise use my
name, voice, and likeness in video, photographs, written materials, and audio-visual recordings.
I acknowledge and understand these materials about or of me may be used for both commercial
and/or non-commercial purposes.

I understand that my image may be edited, copied, exhibited, published, and/or distributed. I
also understand this material may be used individually or in conjunction with other media in any
medium, including without limitation to print publications, digital publications, and/or public
broadcast for any lawful purpose. There is no time limit on the validity of this release nor is there
any geographic limitation on where these materials may be distributed.

I hereby acknowledge and grant “Maskara” and its journalists all ownership rights and the
irrevocable right and permission to use, copyright, publish, sell, distribute, and/or promote the
recorded video, photo, interview, and/or audio.

I understand my participation is voluntary and that I may, at any time, discontinue my

involvement before signing this document. If I choose to discontinue participation, I will notify the
journalists by providing written notice by 9:00 AM on January 12, 2022.

I understand that “Maskara” can see no risk presently and that I take full responsibility for my
involvement in this project and the risks that it may entail (be they legal, physical, or mental) and
release “Maskara” from any claims, demands, losses, damages, suits, and liabilities of any kind
whatsoever in connection with the foregoing.

I hereby certify that I am over eighteen years of age and am competent to contract in my own
name insofar as the above is concerned. If I am under eighteen years of age, my parents or
legal guardians have read this document and have given their consent by signing below.

By signing this form, I acknowledge that I have completely read and fully understand the above
consent and release and agree to be bound thereby. I hereby release any and all claims against
any person or organization utilizing this material for marketing, educational, promotional, and/or
any other lawful purpose whatsoever.
