RE: Notice to Vacate

Dear ________:

This is reference to the lease agreement dated _________ (“Lease Agreement”) that was entered into
between myself and ______________. Last ___________, our Lease Agreement has expired and despite
the Lease of Contract Extension dated __________, your company has failed and continuously fails to
fulfill its obligations therein.

Currently, there are still properties and equipment left by your company and stored in the leased
premises. However, we have not received any remuneration or any rent payment for the same and a
total violation of your obligations as a lessee under Article 1657 of the Civil Code of the Philippines.

Our letter last _______________ requesting ________________________________ has remained

pending and unanswered.

This notice is, thus, to inform you that you have officially abandoned the leased premises and your
presence at _________________________n, is requested to be vacate within 30 days from the date of
receipt of this notice.

You are required to vacate the premises and remove all your possessions from the premises by
__________________. Should you fail to vacate the leased premises within the said duration, will entitle
us to continue with the egress of your equipment on your behalf. All costs pertaining to the egress and
storage shall be at your expense.
