Greetings of peace and prosperity.

In view to the DepEd Order (DO) 012, s. 2021 entitled Amendment to DepEd
Order (DO) 030, s. 2020 on the continuous monitoring of the implementation of Basic
Education in this time of pandemic, teachers are encouraged to address learning gaps
in order to meet the required essential learning competencies through giving
intervention and remediation activities.

In connection, it is my utmost desire to request from your good office to allow me

to conduct intervention and remediation activities as based on the unique needs of my
learners as determined through the results of the different forms of assessment
administered throughout the quarter.

The above-mentioned endeavor will be done through home visitation on March

10, 2021, and in as much as I wanted to give further notice to the parents/guardians of
the students to be visited, some contacts are not available for the reason that these
students have missed couple of weeks in submitting their modules. Thus, I opt to
conduct uninformed home visitation.

Rest assure that proper health protocols will be strictly observed.
