III. Problem Presented

 The client came to our office seeking for any possible financial assistance for his
nephew who was admitted at R.O Diagan Cooperative Hospital due to CLOSED MID
FEMORAL TRANSVERSE FRACTURE RIGHTwith a bill amounting P 31,077.20

IV. Family and Socio Economic Background:

Arman Lumbaran (patient), a student. His mother is working as farmer earning 500/
month. He has two siblings, his father is already deceased. Their harvest is only good for their
everyday needs.

V. Assessment Statement:

Benjamin Mariano (client) is in need of any possible assistance for his nephew Arman
Lumbaranan (patient) to pay the hospital expenses. The family cannot provide the amount of
the hospital bill because their income was just enough for their daily basic needs and no
financial capability.

VI. Recommendation:

Based on the above facts and information gathered. I will recommend that the client
should be given an opportunity to avail an assistance in order to pay the hospital bill.
