Sub: Consent Letter for Face-to-Face (Offline) Classes in the Vidyalaya with respect of my
ward reg.

Respected Sir,
As per the above mentioned subject and as per the Covid-19 Guidelines and Standard
Operating Procedure (S.O.P) issued by State Government (Govt. of Maharashtra)
regarding face to face (Offline) Classes to be conducted in the Vidyalaya
In this regard I give my consent that my Son/ Daughter (Name)
__________________________________ who is studying in your Vidyalaya in Class
________ Section ____Shift ___ in this session 2021-22 will attend the offline classes to be
conducted in the Vidyalaya .
I hereby give my consent that me and my ward will follow the SOP and Covid-19
Guidelines issued by Govt. of Maharashtra and also assuring that my ward will always
wear the mask in the Vidyalaya premises and will carry water bottle and the lunchbox with
him from home and will not share anything with the fellow students and will come to
Vidyalaya as per the time-table of the Vidyalaya.

I will be solely responsible to drop and pick up the ward from the school.

I shall not hold school responsible for any kind of infection due to Covid 19 or any other
Variant .

I also give my consent that if any member of my family became Corona Positive or my
living area will be declared as the Containment Zone, then I will not send my son/daughter
to the Vidyalaya.
