I

Is this Dr. Rosales? I am Nurse Rafaela, nurse on ward. I am

calling about Mr. Enrile, 80 years old female.

S
I am concerned because she is lethargic and respond to
verbal stimuli slowly.

B
She was admitted 2 days ago due to motor vehicle accident,
right arm with cast and will be discharge this morning. During
the night, she fell, with slight nose bleeding.

A
I have just assessed the patient personally, vital signs are:
BP: 90/70 (lower than her usual 130/88), P: 110, irregular
respiration of 14 (shallow), O2 sat: 92%, skin is cool and with
slight cyanosis on nail beds. I think that he is showing signs
of hypovolemic shock.

R
I request that you come and see the patient this time. Is there
anything I should do for the meantime, or anything you will
need as part of your assessment?
