I have just assessed the patient personally, vital signs are:
BP: 90/70 (lower than her usual 130/88), P: 110, irregular
respiration of 14 (shallow), O2 sat: 92%, skin is cool and with
slight cyanosis on nail beds. I think that he is showing signs
of hypovolemic shock.

I request that you come and see the patient this time. Is there
anything I should do for the meantime, or anything you will
need as part of your assessment?
