TEACHING INTERNSHIP AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This agreement is made and entered by and between:

__________________________ with office address at_________________________________,


(Name of Cooperating School), herein represented by _________________________________,
(Name of Principal)
hereinafter referred to as the “First Party”;

–and–

_______________________________, of legal age, Filipino, bona fide student of Cagayan


State University- Andrews Campus and with office address at Tuguegarao City hereinafter referred
to as the “Second Party”.

–WITNESSETH–

1. That the First Party accommodates the Second Party who shall undertake teaching internship for
the period __________________ until____________________;

2. That as a Cooperating School, the First Party is expected to perform the following general
functions:
 design and implement the teaching internship plan in partnership with CSU;
 encourage student interns to develop their personality and professionalism, and to
the extent possible, protect them from physical or moral danger;
 assign a point/focal person responsible for the implementation of all phases of the
teaching internship;
 orient the student interns on the standard rules and regulations of their establishment
before signing of contract;
 enter into a teaching Internship Contract/Agreement with the student intern;
 facilitate the processing of the documents of the student interns in coordination with
CSU;
 provide practical training or work experience in accordance with the agreed
internship plan and schedule of activities;
 ensure that student interns do not perform tasks and duties of regular positions;
 ensure the safety and physical well-being of student interns by providing first aid
kits, safety gears and antidote chart and by avoiding the student interns’ exposure to
hazardous situations;
 provide supervised applied learning experience for student interns in accordance
with agreed Internship Plan and schedule of activities;
 develop feedback mechanism to the student interns;
 develop feedback mechanism to CSU of the overall implementation of the teaching
internship and the student interns’ performance;
 provide monitoring and evaluation reports or other information on the student interns
performance as may be required by CSU;
 make available, accurate and current records and provide access to CSU of such
records while on internship;
 notify and provide at least 30 working days written notice to CSU of student interns
breach of contract or misconduct in the premises prior to DepEd decision to suspend
or terminate the contract;
 officially inform the Dean of the College for any school related activities of the
cooperating school scheduled after school hours or during weekends requiring the
presence of the student interns; and
 issue certificates of completion and grades of the student interns not later than two
weeks after the completion of teaching internship.

3. That the Second Party has signified his intention, to which the First Party has accepted, to
undertake teaching internship;

4. That the Second Party hereby meets the appropriate qualifications set by CHED Memorandum
Order 104 Series of 2017 and Cagayan State University academic policies required to perform
the teaching internship activities as described herein;

5. That in view hereof, the Second Party is hereby considered as a Student Intern for the period
____________until _______________;

6. That the Second Party binds himself/herself to perform teaching internship for the same period
as stated in the preceding paragraph;

7. That the second party hereby agrees to the policies and guidelines stipulated on teaching
internship plan.

8. That as Student Intern, the Second Party is expected to perform the following general
functions:
 enter into an internship contract and/or agreement with DepEd;
 sign all the required documents necessary for his or her participation in the
immersion program, including the acceptance letter, practice teaching/internship
contract and/or agreement;
 comply with the provisions of the contract and/or agreement including the rules and
regulations of the CSU, DepEd and CHED at all times;
 undergo the required orientation conducted by the CSU and DepEd;
 report for immersion based on the schedule indicated in the teaching internship
contract and/or agreement;
 perform tasks and activities indicated in the practice teaching plan;
 maintain confidentiality, when and where appropriate, during and after the practice
teaching period of all the school data where such information is not within the public
domain and is indicated or understood to be confidential;
 adhere to the existing rules and regulations of DepEd including the proper use of
tools, instruments, machines, and equipment;
 submit a journal of practice teaching/internship as required by CSU and DepEd
reflecting on the approved practice teaching plan, his or her experiences describing
the practice teaching activities, any problem/s encountered, and his or her reflections
on the practice teaching experience to the SIPP / Practice teaching coordinator;
 complete the agreed duration of his or her practice teaching; in case the practice
teacher will be unable to finish his or her immersion within the designated period, he
or she shall inform the SIPP coordinator in writing of his or her intent and reasons to
prematurely end his immersion, at least three (3) working days before his last day of
immersion. Failure to complete the practice teaching program without valid cause
disqualifies the practice teacher from retaking the program with the concerned
cooperating school ; and
 report to the SIPP coordinator for an exit assessment after the completion of the
practice teaching period.

9. That as Student Intern, the Second Party is expected to perform the following specific
functions:
 attend student internship from Monday to Friday to complete at least 280 hours;
 attend flag ceremonies;
 prepare and execute at least 15 detailed lesson plans to be checked by the
cooperating teacher;
 prepare instructional materials;
 teach the class formally wearing the appropriate SI uniform;
 assist the cooperating teacher in:
 checking papers
 recording grades
 filling out forms
 preparing grade sheets
 preparing summary sheets
 conducting classroom activities
 preparing visual aids
 managing classes
 supervising “clean and green activities”
 supervising “co-curricular activities”
 supervise examinations (quizzes, periodic exams)
 assist the principal or any administrative staff in any activity that needs their help
(not financial or project funding)
 undertake at least 1 pre-final demonstration teaching to be observed by the Master
Teacher / Principal;
 conduct a final demonstration teaching to be observed by the CSU SIPP Coordinator
and/or Program Chair; and
 undertake a community service.
10. That the Second Party shall perform work at a time and schedule to be agreed upon by both
parties.

11. That it is understood that this agreement does not create an employer-employee relationship
between the First Party and the Second Party, that the services rendered hereunder are not
considered and will not be accredited as government service; and that the latter is not entitled to
benefits enjoyed by the regular personnel of the First Party.

12. That the Agreement, as well as its terms and conditions herein shall be held in strict confidence
and that no portion or part hereof maybe reproduced or be revealed to any personnel or entity
other than the signatories herein or their authorized representative.
