Activity: Accident Investigation

1. What is an Accident Investigation?

Accident Investigation is the methodical gathering and analysis of data relating to an accident that
has resulted in the loss of property, time, or health of individuals, or even the loss of lives, and is carried out
by trained professionals. In other terms, it is a scientific and technological investigation into the
circumstances that transpired during an automobile collision. The purpose of an investigation is to establish
the reason of an accident, after which suggestions are made or corrective action is taken in order to
prevent a repeat of the same or a similar incident in the future. Furthermore, safety professionals with
extensive field expertise are responsible for commencing an accident inquiry when an accident occurs.
These professionals have received the appropriate training and have the relevant competence to conduct
an inquiry into an accident. A unique accident reporting template, which includes the underlying reasons
and recommendations, is used by some organizations or automobile manufacturers. A thorough
investigation may reveal a number of contributing factors, as well as recommendations for preventative
measures.

2. What are the levels of Accident investigation and for each level, how would you apply in the case of
Vehicle Accident?

Accidents are intricate. To effectively prescribe the following preventive activities, substantial
research and inquiry are required. Nonetheless, a thorough study is required, and multiple stage reasons
must be emphasized before getting to the true cause of the disaster. These are referred to as the
fundamental, indirect, and direct causes, respectively. The direct cause is at the bottom of the hierarchy; it
perpetuates the individual who was exposed to the disaster. For instance, in a car accident, the collision
occurred between two vehicles. Additionally, Indirect Cause results in the victim's or suspect's risky act or
situation. In the event of a vehicle accident, it is possible for the suspect to be distracted by their cell phone
while driving, resulting in a collision. Finally, Basic Causes refer to additional external elements that
contribute to an accident. Environmental conditions, traffic situations, unrecognized road signs, and altered
road profiles may all contribute to this.

3. State the Procedure of performing an accident investigation and cite examples to detail each process.

Phase 1: Respond to the Incident

• • Following a vehicle accident, law enforcement officers and emergency personnel must secure the
crash location and take injured individuals to the nearest health clinic. Then, confirm that the scene is
safe to collect the necessary information without causing any disruption.
Phase 2: Gather Information
• •The fundamental objective of data collection is to establish facts that describe the sequence of events
that occurred before to, during, and following the incident and its repercussions. With accurate and full
information, investigators can accurately recreate events and comprehend what occurred. A crash
diagram is created for the incident region, and several elements are taken into account. This
encompasses all components of the physical, human, and organizational realms. Then, photographing
the situation is an efficient technique to collect the necessary facts and question several witnesses on
the spot.

Phase 3: Map the Sequence of Events

• Recreate the incident if possible. a chronologically organized set of events that indicate what occurred
before, during, and immediately after the collision, using the information you have gathered - images,
measurements, statements, your own well-reasoned theory, and relevant data - to indicate what
happened before, during, and immediately after the collision. Prior to a crash, critical decisions are
occasionally made, and significant events take place in the vicinity. If it has anything to do with the
impact, it should be included in the sequence of events.

Phase 4: Determine the Underlying Cause

• The safety officer is responsible for testing and analyzing the hypotheses that were established by the
team in the preceding stages. While testing and analyzing new facts and possible explanations, you
will uncover critical new facts and explanations that will help you move forward in your investigation.
Phase 5: Recommend Corrective Actions
• After a crash has occurred, the major purpose of an investigation is to identify what efforts will be
taken by the organization to avoid future crashes from occurring as a result of the same activities,
conditions, or decisions that led to this one. It is essential that the corrective steps you take address
the basic issues that you have discovered during your investigation.
