updated PRUC License/ID and if expired, proof of renewal
E. Photocopy of Performance Evaluation for those with
teaching experience
F. Photocopy of School Clearance for those with teaching
experience
G. Duly signed service Record of Certificate Employment
H. Certified true copy of Transcript of Records (TOR) with
computed average rating
I. Certified true copy of Certificate of Trainings attended (10
days or more) if any
J. Certified true copy of Proficiency/recognition from
recognized and respectable relevant associations/ organizations
/ if any
K. Photocopy of Voter’s ID / or proof of residency
