Methodology and Clinical Significance: This SARS-CoV-2 test is a real-time reverse transcription polymerase chain reaction (RT-
PCR) assay intended for the in vitro qualitative detection of nucleic acid from severe acute respiratory syndrome coronavirus 2
(SARS-CoV-2). This test is used for diagnostic purposes.

The Food and Drug Administration (FDA) has issued an Emergency Use Authorization (EUA) for this assay for the duration of the
declaration that circumstances exist justifying the authorization of emergency use of in vitro diagnostic tests for detection and/
or diagnosis of COVID-19. This assay has been authorized only for the detection of nucleic acid from SARS-CoV-2, not for any
other viruses or pathogens. This test has not been FDA cleared or approved. Verification of performance characteristics of this

assay were performed by PreCheck Health Services, Inc. that is certified under the Clinical Laboratory Improvement
Amendments of 1988 (CLIA) as qualified to perform high complexity clinical laboratory testing.
A result of “Not Detected” does not rule out nucleic acid concentrations below the limit of the assay or the presence of PCR
inhibitors in the patient specimen. Improper collection and handling can cause false negatives. Negative results do not preclude
SARS-CoV-2 infection. False positives can be caused by PCR contamination. Clinical correlation is indicated to determine patient

infection status.
