Dear Respondent,

We the Group 1 Grade 12 G invite you to participate in a research study about the proposed

innovation about canal suction. We are currently taking up Mixed Method Research at the Sisters

of Mary School-Boystown, Inc. and we are in the process of writing our Research Paper. As part

of our research we are on our quantitative phase to collect data from you.

The questionnaire will be in Google Forms that has been designed to collect information to

evaluate the materials and design of the proposed innovation.

Please do not leave any questions unanswered. Your responses will remain confidential and

anonymous. Data from this research will be kept under lock and key and reported only as a

collective combined total. No one other than the researchers will know your individual answers

to this questionnaire.

If you agree to participate in this study, please answer the questions on the questionnaire as best

you can. It should take approximately 20 minutes to complete. After the given time to answer

please submit your answers immediately.
