5.00 a.m. Mrs. Amanda came to the hospital at 5 am with rather strong uterine
contractions. she is feeling anxious and worried and this is her first pregnancy.
5.15 a.m. Vaginal examination : 7 cm of cervix dilatation, the head of the baby was
notice in the lowest part, no blood slime anymore.
5.30 a.m. Abdominal examination: overall normal, strong contractions, the patient feels a
little pain around the back and a little pain in the lower abdomen.
6.00 a.m. The patient urinates in the toilet independently without assistance. The result:
clear urine and no abnormalities at all.
7.15 a.m. Patient feels contraction increased, the contraction of the abdomen was
3x10’15’’, FHR: 124x/ minute.
7.30 a.m. Called Obstetrician because Patient feels painful an gets strongly
8.00 a.m. Obstetrician have done the USG, the result is amnion water ± 1 L, estimated of
the baby weight its around 3200 gr.
9.30 a.m. Patient feel very painful in her belly, the membran was ruptured and the
amnion water was clear, no blood, no meconium, the smell was normal.
Vaginal examination: fully dilatation. Prepare for the process delivery.
10.00 a.m. The baby was delivered, it was girl baby, normal baby, her weight around 3300
gr the baby crying spontaneously, her skin was reddist.
10.20 a.m. The placenta was born, the perineum intake, the placenta was complete with 18
cotyledons, have no bleeding, blood vessel in the placenta was normal.
12.30 pm Last vital sign BP: 120/ 80 mmHg, P: 84x/ minute, T: 37oC
1.30 p.m. Checked the abdomen, the fundal height is 2 cm under umbilical, the
contraction was strong and long enough, the vesica urinary was empty, she can
put the urine by herself. No rupture for perineum.
