MEMORANDUM OF AGREEMENT

KNOW ALL MEN BY THESE PRESENTS:

This Tripartite Memorandum of Agreement entered into by and between:

The LOCAL GOVERNMENT UNIT OF RODRIGUEZ, RIZAL duly created and existing
under the laws of the Philippines with the seat of local government of Rodriguez Rizal at J. P Rizal Street
Brgy. Balite Rodriguez Rizal, represented herein by. HON. DENNIS L. HERNANDEZ, MAYOR, by
virtue of Sangguniang Bayan

The GENERIKA DRUGSTORE, Head Office and a drugstore chain, with office address at
DPII Bldg., 1st Cervantes Compound, Km. 17 West Service Road Paranaque City.
and
2E PHARMA ENTERPRISES OPC, a franchise of Generika Drugstore, represented by Ivan M.
Masangkay, with office address at 146 M.H Del Pilar St. San Rafael Rodriguez Rizal.

WITNESSETH that:

WHEREAS, the efforts and representation of the LGU, an appropriation in the minimum
amount of 50,000.00 to maximum of 500,000.00 Philippine Currency, has been allocated as financial
assistance for the PHARMACEUTICAL DRUGS/ MEDICINES and OTHER FOOD FOR SPECIAL
MEDICAL PURPOSES of buyer by the GENERIKA DRUGSTORE;
WHEREAS, through the minimum amount of 50,000.00 to maximum of 500,000.00 Philippine
Currency, shall form part of the Medical Assistance Fund (FUND) of the LGU under its Medical
Assistance Program;
WHEREAS, the LGU is sought by numerous indigent and other deserving client/bearer for
assistance regarding their medical condition;
WHEREAS, the GENERIKA DRUGSTORE-Head Office shall ensure that the franchise
drugstore will follow the agreement, monitor the implementation of the program;
WHEREAS, 2E PHARMA ENTERPRISES OPC, a Generika Drugstore franchise
availability of pharmaceutical drugs/medicines and other food for special medical purposes can be
provided to needed indigent and other deserving CLIENT/BEARER requesting for medical assistance
and is willing to cooperate/coordinate in the implementation of the PROGRAM;
WHEREAS, close coordination and cooperation between the parties to this agreement are
essential to the effective and proper implementation as well as monitoring of the aforementioned
PROGRAM;
NOW THEREFORE, for and in consideration of the foregoing premises and the terms and conditions
hereunder enumerated, the parties to this Agreement hereby agree as follows:
1. The LGU thru MSWDO shall identify and recommend in writing to the Generika Drugstore
Franchise (2E PHARMA ENTERPRISES OPC) the indigent and other deserving client/bearer
who may avail of the benefits of the PROGRAM and the extent of assistance that shall be
extended as may deemed necessary. A control number shall be indicated;
2. All client/bearer shall comply with the requirements in availing the program;
a. Present approved and signed Guarantee Letter
b. Present valid identification Card of the bearer including updated prescription of medicine
issued and signed by doctors/physician.
c. Quantity of pharmaceutical drugs/ medicines and other food for special medical purposes
issued shall be in accordance with the prescriptions issued by the doctors/physician.
d. The approved guarantee letter shall be used only once. The actual expenses incurred the
prescription of medicine shall be deducted from the Fund. The remaining balance from the
 amount of the assistance granted shall be automatically reverted to the Fund. Expenses
incurred beyond the amount of assistance granted shall be shouldered by the patient.

3. The GENERIKA DRUGSTORE shall furnish the LGU with liquidation report following the
existing government accounting procedures including the list of client/bearer who have availed of
the PROGRAM including the amount of assistance extended for monitoring and recoding
purposes of the LGU within the 15th and 30th day of every month.

4. The GENERIKA DRUGSTORE-franchisee shall utilize the Fund for payment medicines and ,
but with guarantee that the amount used for the utilization shall be credited to the LGU-
REFFERED CLIENT/BEARER in accordance with the preceding provisions;

5. All payments and releases under this PROGRAM of the LGU shall be made in accordance with
existing government accounting and auditing rules and regulations;

6. No cash assistance from this Fund shall be extended to any CLIENT/BEARER.

7. This agreement shall take effect upon the receipt by the SECOND PARTY of the Program Fund
which shall commence on JULY 2021, until DECEMBER 2022 from the FIRST PARTY or
until the Fund is fully exhausted.
