IU Methodist Outpatient PT Internship Journal

Week 1
1/10/2022
Today was my first day on site at the IU Methodist Outpatient Physical Therapy clinic. I spent
four hours in the morning getting to know the staff and the environment that I would be
working in. One of my lead therapists was able to give me an orientation of my tasks and what
my expectations were. I was also able to shadow her with a couple patients before leaving at
noon.

1/11/2022
During this morning I spent the first few hours with the education director who conducted a
formal orientation to IU Health. We reviewed all of the policies and procedures the hospital has
as well as what would be expected of the interns joining the staff for the spring 2022 semester.
After the orientation I went back to the outpatient clinic and was able to work with some
patients. I met with three new therapists who were not there on Monday, and I was able to
learn from them throughout the day. I also was taught how to clean the cold and hot pack
machines.

1/12/2022
Today was an eventful day and I was able to interact with a lot of patients. I was able to sit in on
a schizophrenic patient as well as a patient with TBI. I also got the chance to lead a couple
exercises during the daily pain clinic. I chose to have the patient do a TheraBand row and a
TheraBand kickback.

1/13/2022
This morning I worked on my journal entries and then once patients started coming in I was
able to go out and help the therapists. Today I was able to work with patients who had knee
and hip pain, neuropathy, back pain, as well as other pains. I also was able to restock all of the
linen throughout the facility during some free time. At the end of the day, I was sitting in with a
patient who was describing low back pain, bilateral pain, night sweats, and trouble urination.
The therapist told the patient that they should refer back to their physician about these
symptoms because it could be more than a musculoskeletal issue. The therapist lateral told me
about the importance of catching red flags like those symptoms because in a lot of cases pain in
addition with other symptoms can be indicators of more serious things such as heart attacks
and cancers.

On Tuesday I started the day with some homework because the first patient was a no show.
After that I watched a therapist do some shoulder therapy including arm bike and TheraBand
work. I also helped out again during the pain clinic and tossed a ball to the patient, held her gait
belt, and collected bean bags that were tossed. Later in the day I observed a patient with back
pain and tightness and the therapist did a lot of soft tissue work as well as some stretching and
strengthening of the QL muscle in the low back. I also observed a patient who had down
syndrome for a little bit.

Reflection
This first week has been a great experience and I am happy to be interning at this facility. I have
gotten to sit in and assist with various patients as well as gel with the staff very well. The most
rewarding part of the day is helping patients with exercises, and this connects with my passion
for physical activity. Physical activity is one of my top priorities and to be able to share that with
others brings me joy. I have been able to help patients walk throughout the facility, perform
exercises like step ups, RDL’s, TheraBand work, parallel bar work, bike exercises, and any other
movements on the tables or equipment. I have also had the opportunity to work with patients
with disabilities. This required me to take a step back and think about what the best way to
treat them would entail.

Week 2
1/19/2022
One of the first patients that I shadowed today was having pain in the shoulder that was likely
due to biceps tendonitis. She also wanted to work on core stability, so the therapist had her do
dead bugs for an exercise. During the exercise he mentioned that she should visualize a string
pulling her lower back into the table, which is a great visual cue to keep her core tight. I also did
some cleaning and watched a patient use the total gym machine for the first time. I sat in on
the pain clinic as usual and helped the patient with their exercises.

1/20/2022
This morning I started off by telling one of my leads about the case study project and asked if
she had any good ideas for that. She told me that a great opportunity would be to follow a
patient who is enrolled in the pain rehab program, which I agreed was a great idea. I then
started making an overview write up of the program and how it would fit the requirements of
this case study. I then shadowed with a therapist who was doing some exercises for knee pain.

He had the patient do some TheraBand work and some step ups. I also helped with some
balance exercises with a patient by holding a TheraBand and tossing a ball back and forth with
her. Before the pain clinic we had our weekly huddle and this week we talked about safety
procedures during an emergency. I also helped lead the pain clinic and had the patient do a
core and balance exercise.

1/25/2022
This morning I worked on my SMART goals and my reflection for week 2. After looking at those I
went around and restocked linen and supplies where things were needed. In addition to
restocking, I put some equipment back where it goes and set up the rooms so that they would
be ready for patients. One of the patients I worked with had some knee pain, so I helped her
with some step ups and wall sits and then hooked her up to the game ready machine. I also got
the chance to work with the patient with down syndrome again and did a ball toss and walking
exercise with him.

Reflection
Week 2 allowed me to get a more concrete understanding of my role at MPC. I used this week
as a chance to really think about what I want to get out of this internship. Outside of general
experience in the field I want to increase my people skills and continue to grow more
comfortable talking to the patients. I also want to begin to build my exercise data base and
know what, why, and when certain exercises are being used. In addition to people skills and
exercises, I also want to continue to grow my love for the field to the point were going into a
geriatric’s facility, for example, brings me joy. I also thought about my core values and how they
relate to this internship experience. My three core values are positivity, inclusivity, and
motivation. I want to portray these traits with all of my patients as well as through the practice
that I will one day work at. Being able to motivate all patients in a positive light is the key to
being a successful physical therapist and I plan to embody those traits throughout my physical
therapy journey.

SMART Goals
1. Continue to build upon my interpersonal relationship skills to the point where I can talk
freely and confidently with 100% of the patients I see.
2. Observe specific exercises and better understand why certain exercises are used to a
point where I can accurately predict what exercises may be beneficial before the
therapists informs me of them 75% of the time.

3. Increase my enjoyment of the profession in a way that allows me to apply my skills in
various areas of physical therapy that were previously not as high of an interest to me
like geriatrics and neurology.

Week 3

1/26/2022
This morning I took my med term test at Butler, so I didn’t get to work until 9, which was an
hour earlier than I planned for. After getting here I restocked some linen and organize one of
the cupboards that had the ankle weights in it. I also cleaned up some rooms after patients left
and removed and put away the game ready after a patient was done using it. I got to help out
with a patient recovering from covid as well as a patient who was trying to transition out of a
powerchair. I also got to help out with another patient who was in the hospital due to covid for
six weeks. This patient was unique because he was experiencing numbness, tingling, and
balance issues post-covid.

1/27/2022
I was able to help out with one of the therapists first few patients this morning. He used a
mirror, coaching cues, and demonstrations to help showcase and perfect the exercises he was
teaching. All of these methods proved to be effective with the patients. I also got to watch how
the traction able works for lumbar pain which is something that I have not seen before. Before
lunch the therapists had their huddle and talked about ways to improve their client’s
satisfaction and one thing they talked about was making sure to ask the patient before putting
their hands on them. Another thing that was brought up was to be transparent at the beginning
of each session and let them know what they would be doing and what to expect throughout
the process. Towards the end of the day, I got to work with a patient who has knee pain and we
worked to get more flexion in the knee.

2/1/2022
Today we had a couple therapists out, so it was a slower start to the day. I was still able to reset
the rooms and restock the linen in the morning while there was just one patient here. This
morning I was able to get quite a bit done on my case study. The patient started the program
on Friday, so I was able to fill in all of the information from the initial evaluation and start
working on a plan of care for the patient. I got to be super involved in the pain clinic and the
client in this one is the person I will be conducting my case study on. I will also be creating a log
of exercises for the program throughout the process. I got to help with a balance patient and a
patient with ankle pain as well in the afternoon.

Reflection
This past week was similar to the last couple, and I have grown pretty comfortable with the
therapists and the routine of things in this clinic. The same things were applicable this week as
they have been in the past, including anatomy and physiology, coaching cues, and coaching
methods. This week I was able to talk to one of the therapists who had just recently graduated
from IU DPT. She talked about her time there and gave me some advice. She also told me the
practical exams are really hard so to be ready for those. It was nice to get some advice and hear
information from a recent graduate and I feel like my relationship with her will only benefit me
through my DPT career.

Future Application
Being in a physical therapy environment is exactly what I need to prepare myself for my future.
This clinic is especially helpful because all of the therapists are so willing to help me learn and
involve me in there. Simple things like understanding exercises, movements, and tests will give
me a boost in my DPT curriculum where we will be taught some of those things. Most
importantly, this internship gives me the opportunity to interact with people in this field.
Interaction with patients and therapist will only benefit me for my future. Continuing to build
my interpersonal relationship skills and confidence in this setting are extremely applicable and
helpful for my future.

Week 4

2/2/2022
Only a couple patients came in today in the morning because of the weather. I had the chance
to put all equipment back where it belongs and restock some of the linen. I got to help out with
a patient who had bilateral ACL tears and then about 2 months ago had a stoke when she was
getting close to be done with rehab. We did a lot of balance exercises like walking on treadmill,
walking and catching a ball, walking with weights, and lunging with weights. I helped out in the
pain clinic again and was able to assists their therapist with some exercises and interaction with
the patient. I also helped out with a patient who had broken their ankle and also had a
condition effecting their nerves.

2/3/2022
All of the patients canceled or were expected to cancel, so I did not go in today.

2/8/2022
This morning I worked on my case study until more therapists got in with more patients. I
elected to formulate the case study as an APA paper. I think this format would showcase the
progression and details the best. I also helped out with am eval patient who was having
shoulder pain and lack of full rom. We did 3 exercises after the eval which were scapular

retractions, front bar raises, and lateral bar raises. After that I cleaned and restocked some
linen. At 11 the pain clinic started and helped design the workouts for the strength day today.

Reflection
This past week was a little hectic with all of the snow and cancellations, but that being said I
was able to work on my case study a little bit and formulate how I am going to do it. I elected to
go with a paper style, and I think I am going to use the APA style format. The beginning
describes the process, patient, and the goals set. Then the middle will give detail on the
sessions I helped out with and what was accomplished. Then the end will include her test
results at the end and indicate whether or not the program was successful.

Week 5

2/9/2022
This morning I was able to sit in on an eval for a patient who had fibromyalgia and was
experiencing a lot of back pain. The therapist talked with her about her symptoms and what all
led up to today. He then did some tests to measure pain and strength. He gave her some
exercises to do at home, scheduled her out, and also recommended aquatic therapy as it is
really beneficial for fibromyalgia patients. I also was able to clean and reset the clinic and
restock the linen. I also helped with the pain clinic again today and it was a core and balance
day. After lunch I was able to help out with a couple patients including one with flank pain, an
ankle and knee rehab, and a patient with a TBI.

2/10/2022
This morning I went through the observation tool and related things I have been doing to the
learning domains and the competencies and recorded them in the key events section below. I
also helped out with a patient who had a rod put in her leg and had very little flexion and
extension in her knee because of a lack of therapy after the operation. We did a lot of manual
therapy and then isometric stretching. We then had our weekly huddle, and the topic was
about our language with patients and how it can affect their outlook on PT and on life. We
talked about how always being clinically correct can scare patients and make them leave
thinking they are worse off than they actually are. I again helped to lead the pain clinic and

demonstrated some exercises with the patient. During the afternoon I was able to work with
patients who had balance problems, knee pain, back pain, and ankle pain.

2/15/2022
This morning there were lectures for the therapists to watch so I had about an hour and a half
to catch up on some notes and other homework. After that I cleaned the hydrocollator and
game ready machines and restocked some linen. I was able to help out with the clinic and also
do some shadowing with a couple different patients including one rehabbing her ankle and a
couple working on their stability.

Case Study
My case study is over a patient in our pain rehabilitation clinic. I am also considering making a
change and instead of just doing the case study over this one patient I’m going to follow
multiple patients and track their progress and then compare them at the end. This would
showcase the effectiveness of the pain rehab clinic for each person, and I could compare and
contrast from there.

Reflection
This week was overall pretty good, I was able to work with a wide variety of patients and sit in
on a lot of different techniques. The pain clinic completed its second week and improvements
are already being seen. The biggest thing I have witnessed is the patient’s realization and
relation of exercise and a decrease in her pain. I was also able to connect content and learning
domains with key events throughout the week. This helped me to connect everything an

continue to strive towards those goals for the reminder of the internship. I also was able to
send those key events and my hours sheet to Dr. Ettl so that he can stay in the loop as well.

Week 6

2/16/2022
This morning wasn’t too busy, so I was able to set up the workout area, we also got some new
stairs in the clinic, so I was able to check those out. I restocked some of the linen, but it was too
empty. I also had time to think about what I want to do for sure with my case study and I don’t
think I am going to do 2 people because I have a lot of information for the 1 patient and feel
like I could present on her. I also helped out with a patient who was having trouble trusting her
knee. I was able to hold a TheraBand so the therapist could watch her perform a slight squat. I
helped out again with the pain clinic and led some of the exercises. After lunch I cleaned the
hydrocollator and then restocked some more linen.

2/17/2022
There were quite a few cancelations this morning, so I took come time to work on my portfolio
and go through a lot of the big assignments and projects that I have done over the past three
years in HMHSE. We also had huddle and we talked about how to provide the best plan for
both of the pain clinic patients. One of the pain clinic patients wasn’t able to come in today so
we just had one. I hadn’t worked a lot with this patient, so I spent some time getting to know
her. She said this morning her pain was around a five out ten and was having some muscle
spasm

2/22/2022
This morning I came in and restocked all of the linen which was almost depleted. I also cleaned
up some of the clinic and put things away. I also went through he old TheraBand’s and threw
away the ones that were stretched out and then made new ones to use. We then found a bed
bug in one of the rooms, so I was able to help trap it in a cup and then block off the room,
which made for an interesting morning. Dr. Ettl is going to FaceTime in for the pain clinic today.
Before the clinic I helped out with a patient who had knee pain and numbness as well as a
patient with back pain. I also was able to help a therapist with a patient who had trouble
trusting her left knee through a squat pattern. Dr. Ettl was able to facetime in for the pain clinic
and watch me help out with it. After lunch I helped out with a couple different patients and
cleaned as usual. I also got to work with our patient with down syndrome and that was a lot of
fun.

Reflection
This week was a pretty good one and I was happy to see the patient complete this cycle of the
pain clinic. She said she might schedule out follow up appointments because of how much of a
difference she is noticing. I was happy to narrow down what my final case study was and all I
need to do now is put the finishing touches on that project. I have also been able to get a good
chunk of building my website and attaching lots of files for the important projects, papers, and
assignments that I have completed through my time in HMHSE. It was cool to go back and look
at all of my work and see how they are connected the standards and to each other.

Week 7

2/23/2022
This morning I restoked the linen and cleaned up and organized the treatment area. After that I
made a bunch of folders to give to patients on their eval days. I helped out with the pain clinic
as usual and made some exercises for the patients. I studied for my med term exam a little in
the afternoon and also helped with a few patients.

3/1/2022
This morning I cleaned up the exercise area and restocked all of the linen. We also got a new
wheelchair the came in today, so I put that together. I helped out with normal cleaning
responsibilities and aided the therapist with whatever they needed. I also got to sit on a dry
needling session, which was something I had not seen before. This was a really cool experience,
and I was able to learn a lot. As usual I helped out with the pain clinic and got to lead a few
exercises. This afternoon I got to help out with one of our patients who has down syndrome
and I really enjoy being with him.

Reflection & supervisor prompt


This past week only amounted to two workdays instead of the normal three days
because I visited my girlfriend’s family for a long weekend. The highlight of the week was
definitely the dry needling experience that I was fortunate enough to get. I had never got the
chance to witness it before and the therapist was able to talk through why she was doing what
she was doing. I also was able to work with our patient who has down syndrome which is
always a positive because he is a favorite of the clinic.
I feel comfortable asking any of the therapists on staff for a letter or recommendation
and I believe that all of them would have a similar write up. I spend more time with a couple
therapists so they may be able to give more detail as to what my capabilities are. I think that
the letter would talk about my willingness to learn, my communication skills, and my
professionalism in the clinic. I would say those are my three strengths in this internship and I
believe they are visible while I am on site. There haven’t really been any critiques brought to my
attention, but if there were to be I would do whatever it takes to correct those measures to the
best of my ability. Most of my questions during my internship relate to procedures with
patients. This past week I was able to watch a therapist perform a dry needling procedure on a
patient. This was a completely new experience for me, so I was able to ask questions about
placement of the needles, methods with the needles, and benefits of the procedure. I was also
able to get input directly from the patient which was beneficial to my learning.

Week 8

3/2/2022
This morning I had my second exam in medical terminology in the morning, so I didn’t get into
my internship until around 9am. Once I got in, I went over the observation tool with my

supervisor, and she went through why she graded me the way she did in certain scenarios and
gave me some ideas for continued growth. After that, I restocked linen and cleaned up the
exercise area and then helped out with a couple patients. I also asked a couple of the therapists
about some loan information and possibly scholarships through grad school which was a bit
intimidating thinking about the finances, but it was also helpful to get so other points of view
on how to handle it in the best way. As usual, I helped with the pain clinic and lead a couple
exercises for the patient. During the afternoon I was able to work with one of our patients who
has a TBI and is also a favorite of the clinic. He brought us all candy and it was the highlight of
my day.

3/3/2022
This morning on of the therapists bright in dunkin donuts for everyone so that was a pretty
good start to the day. In the morning I was able to sit in with a patient who had a torn PCL and I
got to see her posterior lag which was very interesting and cool to see. I helped with the pain
clinic again and the patient today was not feeling great. She had a pretty high pain night and
morning so far. In the afternoon I was able to help out with a couple patients. I also got to use
the game ready machine on my knee and it felt really good.

Week 10

3/15/2022
This morning was my first time back in the clinic in over a week, so I spent some time restocking
and cleaning everything in order to get it back into the proper shape. I also talked with the
therapists about my break and how the last week went. I was able to help out with a patient

who was working on balance exercises. I was also able to help out with a few other patients and
therapists. I was also able to see a dry needling practice again, which was really cool to see.

3/16/2022
This morning I signed some paperwork for my lease starting this summer and then I restocked
some linen and cleaned up some of the treatment area. Throughout the day I was able to be
with a couple therapists during their treatment sessions. I also organized one of the back rooms
and filled a bunch of paperwork so that evaluations are easier for the therapists.

3/22/2022
This morning I worked on my portfolio for a little bit. And then helped out with a few patients. I
was able to work on a balance activity with a patient. I also helped out with the pain clinic, and
we had two new patients today. After lunch I sat in on an evaluation and watched the therapist
do some manual therapy on the patient. One of my coworkers from the wound clinic came to
say hi to everyone so he was good to see in the afternoon.

Week 11

3/23/2022
This morning we had a new therapist start. I was able to stock the linen and clean everything
before helping with any patients. I was able to assist the therapists with a few patients
including some with back pain and knee pain. I helped out with the pain clinic again and was
able to choose and lead a few exercises.

3/24/2022

This morning I was able to help with a patient as soon as I got into the clinic. I helped this
patient with some step ups and gait training exercises. After that I took some pictures around
the clinic to add to my portfolio and presentation and then I started putting some things
together for it. After that some of the other therapists got there and we had our weekly huddle.
I then helped out again with the pain clinic and I got to pick a couple exercises for the patient to
do. At the end of the day, I got to watch a therapist do some dry needling on a patient which is
always cool to see.

3/29/2022
There were only 3 therapists in today and the pain clinic patients were both sick, so it was a
slower start to the day. I was able to help out with a patient who had low back pain. I restocked
all of the linen and organized the gym during down time. I was also able to have a little photo
shoot and get some visuals for my portfolio and final presentation. After lunch the IU DPT ICE
students came in so I talked to them for a while about how their first year is going and what I
should expect heading into it.

Reflection
This week I was able to put together some of my eportfolio and link the internship and
competencies to classes I had taken in the past. This was actually somewhat enjoyable because
I got to reminisce a little over the past 3 years in HMHSE and the past 4 years at Buter. My
internship was about the same as other weeks and I got to help out and give suggestions to the
therapist about what to have the patients do. It is also super helpful to be able to get insight
from current IU DPT students when they come in for the ICE rotations. Being able get their
point of view after their first year is almost over with is really beneficial and helps to set my
expectations for what’s to come.

Week 12

3/30/2022
This morning I worked on cleaning the gym and restocked some linen. I also did some clerical
stuff and made copies and handouts for the patients. After that I went through the TheraBand
drawer and threw away the old and worn out ones and then replaced them with fresh ones.

3/31/2022
This morning I did my morning routines of cleaning organizing and stocking. I also helped out
with a patient and did a little race with him and did ball tosses. We have our weekly huddle this
morning and we talked about how to properly recognize the PTs for their accomplishment and
what they want/aren’t getting from management. After that I helped out with the pain clinic as
usual.

4/5/2022
This morning I restocked and cleaned everything from the last couple days. Before lunch one of
the therapists needed my help in the ED so I was able to be with her over there for a few hours.
I had never been to the ED, so it was really cool to experience that. I was able to help out with a
couple wounds patients, so it was nice to have the prior experience from my job.

Week 13

4/6/2022
This morning I had my third med term exam, so I got to work after that around 9. Once I got
here I restocked and cleaned up what needed to be cleaned up. After that I worked on my
competencies and picked out which ones I’ve accomplished during observations and which

ones I still need to accomplish for Dr. Ettl. I helped out again with the pain clinic and gave some
recommendations on some exercises and ways to downward extended some of them. We did a
couple functional exercises since the patient was experiencing some pain and wanted to know
how to alleviate that through her daily activities.

4/7/2022
This morning I restocked some linen and cleaned up some of the gym area and the tables. One
of the PTs brought in doughnuts for my birthday so that was really nice. Throughout the day I
completed my normal activities and was able to help out with a few different patients. Dr. Ettl
also called in and watched me sit in on an evaluation. After his call I was able to debrief with
him about what he saw and what I still need to complete for him.

4/12/2022
This morning I was able to reset the gym and restock the linen so that it was all ready to go for
the day today and the week ahead. After that I helped out with a patient who was having
trouble with her AFO. I was able to assist in some balance exercises with her. I also helped out
with a patient who had thoracic kyphosis. After lunch the ICE students came in so the clinic was
pretty busy

4/13/2022
This morning there were a couple cancelations, likely due to the weather so I had some time to
do some studying. After some patients showed up I was able to sit in on a pre-op TKA patient.
The therapist gave her some insight on what to expect and how they would start therapy after.

After lunch I went around and restocked all of the linen throughout the clinic. During the
afternoon I was able to help out with a few of our reoccurring patients.

4/14/2022
This morning I was able to fix some things on my presentation. I was able to help out with a few
patients throughout the day as usual. I also made a bunch of eval folders for the therapists to
give to patients after they see them for their evaluations. In the afternoon I watched a therapist
do some manual therapy on a patient who had a some plantar fasciitis and posterior tibial
tendonitis.

4/19/2022
This morning I restocked everything from the weekend and set up the gym, so everything was
ready for the patients and the therapists. I helped out with one of the patients who was having
balance problems. I then called Dr. Ettl, and he was able to sit in on a PAO eval with us. He
watched me demonstrate some things for the patient. In the afternoon the ICE students came
in like usual on Tuesdays.

4/20/2022
This morning I didn’t need to come in until noon because the therapists had conferences in the
morning. They got pizza for lunch to celebrate my last week which was very nice of them! In the
afternoon I went to the rehab office and turned in my internship booklet and badge to the
intern coordinator.

Today was my final day at my internship and it was a pretty good day. One of the therapists
brought in some homemade Reece’s cupcake/cookie treats. During the day I did my normal
activities and helped out with a few patients. Overall it was a great end to a great semester and
I really enjoyed myself at this internship!
