EVALUATION GUIDELINES:

This evaluation form is to be completed at mid-term (left-hand side of page) and at the end of each
placement (right-hand side of page).

The placement mentor is asked to reflect on the student’s current level of performance in each
competency area as related to the provincial standards revised by the Ministry of Training, Colleges
and Universities in 2012. Sheridan students will be evaluated in the following competency areas

1. Perform Professionally
2. Build Caring and Nurturing Relationships
3. Maintain a Safe and Healthy Learning Environment
4. Observe, Document and Reflect on Children’s Play and Learning
5. Plan and Implement Responsive, Play-Based Learning Experiences
6. Promote the Development of the Children’s Well-being

Students are required to complete their own copy of the form INDEPENDENTLY; their self-evaluation
will facilitate discussion and clarification of performance evaluation, and should be discussed in
detail, and signed by both student and placement mentor. This form does not grade students; it
describes their skills and abilities.

The final grade for the course is the responsibility of the Sheridan field visitor/professor.

Competency Guidelines:

For each competency area please indicate the student’s overall performance level:

Areas of strength
Overall I think my professional performance is good. I show up to placement on a weekly basis at the same
time, I seek feedback and handle the feedback professionally, I am respectful of the children and their privacy
as well as the staff, and I am keeping my work life strictly work.
Areas of professional growth
I would like to work on taking more initiative, learning more songs and initiating more during transitions with the
children.
Final
Areas of strength:
I think that I have a good professional performance. I have worked on being more punctual and showing up on
time and taking my breaks at a consistent time. I respect the rules and policies of the centre and take a lot of
initiative within routines and transitions.
Areas of professional growth:
I think I can work on doing weekly planning forms and staying consistent every week with submitting my forms.

Areas of strength
Overall I do think I have done a good job at building caring and nurturing relationships with the children
through interacting in a supportive manner and supporting their individual needs, active listening and positive
respectful communication skills.
Areas of professional growth
I think I can work on expressing my needs and boundaries more and initiating more communication outside of
my classroom with parents once Covid restrictions are lifted.

Final
Areas of strength
I am still maintaining those relationships with the children and building an even stronger bond with them on an
individual manner. I have worked on interacting more with parents when I get the opportunity to since Covid
restrictions have been lifted. I have also worked on demonstrating positive communication skills.

I want to continue to interact with parents and other classrooms and teachers in the centre more for field 3.

in supervision and helping assist children.

environment by ensuring materials that I bring in meet safety standards. Participated in 1 fire drill.

I pay close attention and give lots of detail to the children and add onto their play experiences to learn the
most about the child and their individual interests to create meaningful learning stories and implement
activities that they will enjoy.

I believe I have made effective and appropriate observations towards each child that I have observed and
have implemented appropriate activities.

I want to continue to build on this skill of implementing ideas and continuing to observe each child and
providing them with meaningful activities.

I have made 9 learning stories using the observations I have made of all the individual children and
implemented engaging and appropriate activities. I use the activity implementation as an opportunity to
engage with the children on deeper levels.

I support children in all of the ways I can and demonstrate prosocial behaviours and I try to model appropriate
behaviours for them.

I would like to work on using constructive language and anticipating challenges ahead of time.
