For today I will be doing the return


demonstration for catheterization.

1. Check the doctor’s order for catheterization (catheterization is a dependent nursing action)
2. Explain and discuss the procedure to the patient. (to ensure that the patient really understands
the procedure and to give an inform consent)

ang order ni doc nga insertan ka og catheter. Pasabot niini ma’am kay taudan nato og
tubo imong kinatawo arun mapagawas nato ang imohang ihi og arun mamonitor sab
nato kini. Makabati gyd ka og sakit ani nga procedure ma’am. Okay raba nimo
ma’am nga buhaton nato ni? Naa kay mga pangutana?
3. Perform hand hygiene (to prevent the transmission of infection)
4. Arrange the equipments needed for the procedure
- Dispense the lubricating gel into the kidney basin
- Pour cleansing solution over the cotton balls
- Fill in the 10cc syringe with sterile water
- Cut micropore
5. Provide privacy to the patient by closing the curtains or doors. If curtains or door are not
available use a blanket to cover the patient’s private part. (to prevent the patient of feeling
embarrassed and to maintain dignity)
6. Ask the patient to remove underwear
- Ma’am hubuon na nato imong panty ma’am ha. Alsaha lang imong lubot ma’am.
7. Position the patient depends on the gender. (patient should be comfortable, with perineum or
penis exposed to have accessibility)
Female
- Position the patient in a dorsal recumbent position which is on the back with knees
flexed and thighs relaxed)
Male

- Position the patient in a supine position with legs extended and slightly separated
8. Ensure that a good source of light is available (this will increase the accuracy and speed of the
procedure)
9. Apply sterile gloves using a strict sterile technique
10. Place a waterproof sheet under the buttocks of the patient

11. Connect the urine bag and catheter
12. Clean the genital area of the patient.
Female
- Clean both lablial folds and then the meatus using a single stroke

Male

- Clean in a circular motion from the meatus without retracting it


13. Lubricate catheter ½ inch from the tip
14. Place the thumb and the index finger on the labia minora, spread it and pull upward.
15. Insert the catheter
Female: 2-3 inches Male: 6-7 inches
- Mag taod nako sa catheter ma’am ha ginhawa og lalom ma’am
16. Hold the catheter securely while bladder empties or until urine flows
17. Inflate the balloon.
18. Pull gently until resistance is felt
19. Secure the catheter to the patient’s thigh
Female: secure catheter to inner thigh
Male: secure catheter to upper thigh
20. Cleanse and dry the perineal area with cotton balls
21. Do an after care of the equipments (dispose the equipments used)
22. Remove gloves
23. Perform hand hygiene
24. Document the procedure and the urine output

Removal
1. Check the physician’s order for catheter removal
2. Perform hand hygiene and wear clean gloves
3. Provide for patient’s privacy
4. Explain the procedure to the patient

5. Remove catheter and dispose properly
6. Remove gloves and perform hand hygiene
