California PBIS School Recognition System Application:

2022
The PBIS School Recognition Application process recognizes schools for fidelity of PBIS
implementation and for achieving valued outcomes at the Bronze, Silver, Gold, or Platinum
levels. For your school to be considered for recognition at any level, please complete the
application fields below with your school's information. Fill in and check off the appropriate
boxes for the level for which you are applying. Complete instructions and requirements are
available at: https://pbisca.org/recognition-guidelines

1. Please note that for the purpose of recognition, your team must complete the Tiered
Fidelity Inventory (TFI) with an Authorized CPC External Reviewer (ACER) and the TFI scores
must be entered into PBIS Assessments at PBISapps.org. The TFI is required for ALL
LEVELS of recognition. Information and instructions for the TFI can be found at:
www.pbisca.org. For the purposes of Recognition, the TFI must be completed during the
current school year.

2. In addition to the specific data required for your recognition application, your team will
need the following information to complete the application:

a. School Name (along with county and region information)

b. District Name

c. School Administrator Name and Email

d. School Based Coach and Email

e. Contact Person's Name and Phone Number

f. External Coach's Name and Email

3. All completed applications must be submitted by May 13, 2022 at 11:59pm (note that the
original date had been May 6th)
