The undersigned public attorney is the counsel of accused ARNOLD SAN JOSE TANTAY (the
“client”) in the case entitled People of the Philippines vs. ARNOLD TANTAY SAN JOSE, Criminal Case
No. 64012-19.

Please be informed that our client has a pending Application for Probation (“application”) with
the PAROLE AND PROBATION ADMINISTRATION (hereinafter “Probation Office”). One of the
requirements for said application was a copy of his birth certificate.

In connection hereto, we would like to request for ONE (1) copy of Certificate of Live Birth of our
client to enable the Probation Office to process his application.
