As the Hill State Hyundai project is a multi-storied project, which is Apave have a
methodology:

II. METHODOLOGY FOR VERIFICATION AND CERTIFICATION
OF SATISFACTION OF CONDITIONS FOR SAFETY OF FORCE
BEARING CAPACITY
The services to be provided by APAVE within the scope of works of this proposal include:
- Verification of compliance of the technical design
- Technical control of the project construction & installation
- Certification of satisfaction of conditions for safety of force bearing capacity.
The verification and certification of the compliance of the technical design for the
purpose of certification of satisfaction of conditions for safety of force bearing
capacity of the project in accordance with applicable law and regulation of Vietnam
shall be done by verification of legal profiles of the design documents and design
review reports done by the company reviewed the design. The verification and
certification of the compliance of the technical design done by APAVE is limited within
the scope of safety of force bearing structure capacity of the project.
Should APAVE have any doubt on the compliance of the design documents in
comparison with the applicable codes and standards for design, APAVE will ask the
client / investor to clarify with the design company. APAVE may review the material’s
calculations of main structures if it is necessary for certification of satisfaction of
conditions for safety of force bearing capacity of the project.
To assess the compliance of the technical design documents for the purpose of
certification of satisfaction of conditions for safety of force bearing capacity of the
project, the following contents shall be verified by APAVE:

Verification of application of design codes and standards in comparison with
client’s / investor’s requirements and other applicable rules and regulations,
Verification of format of engineering documents in comparison with
requirements of engineering codes and standards,
Verification of design review, design appraisal and approval done by
engineering company and design review company in comparison with
requirements of relevant regulations on management of quality of design.
The verification shall be performed on the basis of review of the design review
reports done by the design review company for the following project items. During
the verification process, should APAVE have any doubt on the compliance of the
design documents in comparison with the applicable codes and standards for
design, APAVE may review the material’s calculations of main structures if it is
necessary for certification of satisfaction of conditions for safety of force bearing
capacity of the project.
The report on design review and approval of the following project items shall be
verified:
Design of piles and foundation structures of the project,
Other underground structures and basement structures, if applicable,
Design of columns, girders / beams, slabs and roof of the project,
Design of boundary walls, load bearing walls and other superstructures
intended to receive permanent, climatic and operational loads and transmit
them to the foundations,
Design of racks and other load bearing structures for technical systems, such
as electric cables, water supply and drainage, air conditioners and lifts,

APAVE shall inform client / investor all design documents to be verified for the
purpose of certification of satisfaction of conditions for safety of force bearing
capacity of the project.
Before verification of compliance of the design for certification of satisfaction of
conditions for safety of force bearing capacity, the client / investor shall provide
APAVE with a list of all design documents developed by Design Company for the
project. The list of design documents should include the following minimum
information:
Name of design document
Reference number
Revision number (issue number)
APAVE will review the list of documents and register of document levels to be
verified by APAVE for the purpose of certification of satisfaction of conditions for
safety of force bearing capacity of the project. Type of design documents to be
provided to APAVE for verification includes:
Documents for information – I: The documents provide information and data
to be used by APAVE as basis for verification, comparison and assessment of
the compliance of the design. This type of documents may include soil data
report, FEED, owner’s design specifications, list of applicable codes and
standards used for design, etc…
Documents for verification and certification – A: Documents which were
developed on the basis of design specifications and requirements of
applicable codes and standards. This type of documents may include
drawings, calculation notes of structures and specifications of materials, etc…
Design review and appraisal reports done by design Review Company.
The list of design documents which has been marked up by APAVE on the above
basis shall be sent to the client / investor for registration and provision of the
design documents for verification and certification by APAVE.
Whenever the registered list of design documents is updated, such updated list
should be sent to APAVE for review and re-registration.
Based on the registered list of design documents for verification, the client /
investor should transmit such registered documents in completed set or packages
to APAVE for verification.
Upon receipt of the registered design documents, APAVE will review the
documents to ensure that they include all necessary information for verification. If
there is any information missing that the design verification can not be performed,
APAVE will contact client / investor for submission.

The design verification shall be performed by professional engineers on
architecture and structure in accordance with the scope of works as indicated in
the above Paragraph 1.2.
When the design documents are changed / revised according to the design
verification report issued by APAVE, such revised documents should be sent to
APAVE for re-verification and certification.
Review of design documents by APAVE is done with two issues for each
registered design document: origin version for verification and comment, and one
revised version for verification of incorporation of APAVE issued comments.

The technical control of the project structural items which have been constructed
shall be executed by means of verification and assessment of quality records
issued by the constructor and approved by construction Supervision Company
and client / investor. The scope of technical control includes:
The verification is to ensure that the constructor has used the latest and
approved design documents for construction. The verification includes the
following activities:
Verification and comparison of the list of design documents used
by the constructor and the list of design documents verified and certified
by APAVE,
Verification of approval status of the design documents in
comparison with the list of verified and certified design documents of the
project.

inspection and acceptance issued by the constructor in comparison with
actual status of the construction of the items.
The verification is to ensure that the constructor has used the latest and
approved design documents for construction. The verification includes the
following activities:
Verification and comparison of the list of design documents used
by the constructor and the list of design documents verified and certified
by APAVE,
Verification of approval status of the design documents in comparison with
the list of verified and certified design documents of the project.
Before and during the process of production of ready mixed concrete, the
following aspects will be inspected, if it deems necessary for the certification:
Review of quality control plan of batching plan,
Review of approval mixed design of concrete
Review of material test reports of aggregates, cement, water and
admixture,
Review of physical and chemical analysis reports and compressive
strength of concrete according to mixed design,
Verification of quality control activities of batching plan by means of review
quality records and monitor of concrete quality on site.
Steel and other materials used for construction of load bearing structures of
the project shall be inspected on site at random basis before construction.
The scope of inspection includes:
Review of manufacturer’s material certificates
Visual inspection of grade, condition and specification of construction
materials on site,

Witness of sampling of materials for testing in laboratory in accordance
with applicable codes and standards,
Review of material test reports to confirm its compliance with the design
specifications.
Before construction:
• Review of construction method of statement, quality control procedure
and inspection and test plan used for piling, inspection and
acceptance of piles,
• Review of pile fabrication dossiers of the fabricator,
• Visual inspection of piles,
• Inspection of compressive strength of concrete and location of dowel
bars and reinforcement rebar by concrete test hammer and ultrasonic
testing equipment, if it deems necessary for the certification,
• Inspection of reinforced cages works for bored piles, including grade,
dimension of steel bars and cage reinforcement,
• Verification of force capacity of hammers in accordane with filing
design requirements and status of other test piling equipment used for
controlling parameters.
Pilot pile construction:
Depending on piling application for the project, the technical control of
piling works may include the following activities:
• Check bore dimension and depth,
• Check reinforcement cage,
• Check adequate mixed concrete by review of batching sign off sheet,

• Check concrete slump and temperature at site,
• Witness of sampling for test of compressive strength,
• Check dimension and length of pile,
• Check piling refusal parameter,
• Witness static load test of pile,
• Witness PDA (pile driving analyzer) test of pile,
• Witness PIT (pile integrity tester) test of pile,
• Witness ultrasonic test of pile,
• Review test results of compressive strength,
• Review pile driving and inspection records.
Mass piles construction:
Depending on piling application for the project, the technical control of
piling works may include the following activities:
• Check weld joints of piles,
• Check pile depth,
• Check pile refusal,
• Check bore dimension and depth,
• Check reinforcement cage,
• Check adequate mixed concrete by review of batching sign off sheet,
• Check concrete slump and temperature at site,
• Witness of sampling for test of compressive strength,
• Review test results of compressive strength,
• Review pile driving and inspection records.
The technical control shall be performed with force bearing structures, which
include columns, girders, slabs and roof of the project. The technical control
includes the following scope of works:
Inspection of reinforced steel:
Review material certificates and specifications against the design
requirements, including
• Review material certificates to ensure that only materials which meet
designed requirements can be used for the construction,
• Check the construction of reinforced steel, including number of bars,
dimension / sizes, forming / spacing,
• Check weld joints, connections, overlaps of the bars and structures,
• Check dowel bars, anchorages for coming reinforcement concrete
elements and other embedded items for mechanical and electrical
items, etc.

• Check verticality, level, alignment and securing of reinforced structure.
Boundary walls, load bearing walls which are required for safety of operation
shall be inspected during construction. The scope of technical control includes
the following activities:
Review material certificates according to the designed
requirements,
• Type, grade and dimension of the materials,
• Analysis results / test reports of materials
• Grade of mortar
Technical control of construction and inspection for acceptance
• Number and type of dowel bars
• Setting out and alignment of blocks
• Key bonding and Vertical joints’ widths of blocks

• Perpendicular / verticality and solid of block
• Review quality records for construction and inspection of block works
- Technical control of erection and installation of steel structure, racks and other
load bearing structures for technical systems, such as electric cables, water
supply and drainage, air conditioners and lifts

b. Random inspection on site:

If there is any inspection result not meet the requirement of the design
document and/or applied standard, APAVE surveyor will issue Non-
Conformity Report with the following details and transmit it to the Client:
Materials, project items / sections not meet the requirement
Drawings, reference documents
Description of non-conformity
Photos
The quality records issued by the Constructor were verified and checked by APAVE
Surveyor (s) during site construction surveillance. For the purpose of the certification, the
final checking As-Built documents shall be performed on random basis with the following
documents:
- As-built drawings of project’s items and whole project
- Laboratory test reports for main materials, including steel & compressive strength
of concrete.
- Inspection report for in process inspection and final inspection for acceptance of
the project’s items
- Final inspection for acceptance and handover of the project
- Changes / revision to origin approved design documents.
4. Meeting on site
a. Attend a meeting
As required by the Client / Constructor APAVE project manager and/or surveyor will
attend meeting to discuss on quality project which are falling the APAVE scope of work
for assessment and certification of the project.
APAVE will not attend a meeting organized for discussion between Client/Investor and
other concerned parties on any other matters which are not in the scope of APAVE for
assessment and certification of the project.
b. Convene a meeting
If it is required for discussion and settlement of any matter in connection with
construction techniques, quality of materials and quality of construction, APAVE project
manager or surveyor may ask the Client/Investor to convene a meeting for discussion
with related parties on the non-conformance issue.
APAVE project manager / surveyor will made minutes of the meeting for signature of
the concerned parties to confirm the decision of the meeting.
a. On spot reports
These reports, in case of any, may include:
- Site notice
- Non-conformity report
b. Monthly report

This report summarizes the findings of technical control of the construction on site
during a week. Monthly report includes the following main information:
- Main contact of client / investor / constructor and other concerned parties
- Summary of control activities done by APAVE
- Comment on quality and compliance of materials and project’s items which are
controlled and inspected for acceptance
- Summary of Site Notice, Non-conformity Report issued
- Summary of outstanding issues which required an action from the constructor,
including Site Notice and Non-conformity which are still opened
- Control activities to be done next week
- Other issues on quality
- Recommendation to constructor / client / investor.
A Monthly report will issue to client with a copy to relevant parties in the next week.
c. Final report
After completion of assessment and certification services as agreed in the contract for
the services, APAVE will issue a Final Verification Report to summarize all the
assessment activities on the project. The Final Verification Report includes the
following main contents:
- Short description of parties involve in the project, it may include investor,
constructor, designer, project management and project surveillance companies.
- Description of the project
- APAVE scope of assessment and certification of the project and exclusion to
scope of work, if any.
- Reference documents which are used for the services, such as Contract for the
services, design documents, applied codes and standards, etc
- Summary of verification and inspection activities done by APAVE, including
design review, verification of materials and equipment, technical control of the
construction
- Comment and qualification of APAVE on the quality of construction of the project,
if any
- Appendixes attached to the Final Verification Report. Which may include a list of
applied code and standards and other reference documents, a list of approved
design documents, a list of design review reports, list of inspection report, site
notice, non-conformity reports issued during the construction, etc...
- Final conclusion of quality of the project and recommendation for certification.
After the completion of design review and technical control on execution as well as the
corrective action (if any) having done by the contractor, APAVE will grant the certificate on
weight bearing capacity to the building according to items stipulated in Circular
16/2008/TT-BXD on September 11th, 2008.
With the certificate signed by APAVE, the owner will enjoy the following advantages:

- Be legally inline with the stipulation noted in Circular 16/2008/TT-BXD by the Ministry
of Construction, Decree 209 by the Government on Construction Quality Management

- Be appreciated in leasing, transferring, or quantizing
- Reduce the fee of repairing, maintenance and correction in the process of operation.
1. Scope of work
It includes M& E works and landscapes work and Architecture work/finishing work

APAVE surveyors will perform surveillance on M&E works to assure the compliance with
approved design requirements. The details scope of works includes the following main
activities as and if applicable to the project:
2. Performance
To ensure that APAVE can perform its service satisfactorily, Client undertakes to supply it
with details of the performance conditions required for contractors, specifications and
working drawings, and design calculations for dimensional characteristics.
The Client will provide APAVE with the results of such tests and measurements, so that it
may assess them against the contractually stipulated performance levels known to it.
3. Special provisions
In performing its service, APAVE is not responsible to perform the measures of the
performances but to witness the said measures to confirm their validity.
APAVE services do not include the review of the design and calculations of the equipment
and will not provide comments on the design and calculations associated to these
equipments.

Upon completion of the verification of process equipment performance, APAVE will issue a
report on status of operation of the process equipment. The report also indicates
outstanding issues, if any, of equipment which require corrective actions.

Execution plan for commissioning and performance tests
APAVE will verify and witness of commissioning functions to ensure that the Project meets designed
specifications. APAVE will record deviations or potential for such deviations for final reporting.
a. Pre-commissioning checks
All relevant activities and results shall be duly recorded on appropriate check list and
log sheets for works traceability.
The involvement of APAVE Supervisor (s) for this phase will be as follows:

The commissioning activities are associated with running or operating the equipment.
The commissioning includes:
- Generating sets:
Check pre-commissioning documentation
Check running tests of the generating sets
Check the running of monitoring equipment
- Transformers:
Check pre-commissioning documentation
Check operation of protective devices

Check the transformer under voltage, the transformer load
- Motors:
Check pre-commissioning documentation
Check protective devices
Witness running tests
Running during process trials
Check consumed power (to approved drawings)
Inspect for abnormal noise, over heating, vibrations,...
- Power distribution:
Check pre-commissioning documentation
Check operation of the Power Distribution Control system (including interfaces
with connected installations, if any)
- Lighting circuits:
Check pre-commissioning documentation
Emergency lighting to be checked at night (simulation of an electrical shutdown)

Execution Plan for audit of as built documents
After completion of the commissioning and performance test, the surveyors of the
Certification Agent will carry out a final Audit of “As Built Documentation”. The as-built
documents to be audited by APAVE include:
- Shop drawings,
- Operation procedures,
- Handover acceptance tests report / certificates
This final audit shall verify the compliance of the completed works with all the requirements
of the Project contract, codes of practice and standards stated or implied in the Project
contract. NCR’s shall be closed out prior to the sign off of the as-built document and
issuance of the Final Certification for the Project.
APAVE will sign off on front page of each package of as-built document that has been
audited by APAVE.
