VERIFICATION & CERTIFICATION

I, ELLEN FLORES GREGORIO, legal age (48 years old), married, a Filipino
citizen, nail technician and a resident of Zone II, Cabaroan, San Fernando City, La
Union, Philippines, after having been duly sworn to in accordance with law do hereby
depose and state that:

1. I am the Petitioner in the above-entitled case;

2. I have caused the preparation and filing of the foregoing PETITION;

3. The allegations in the PETITION are true and correct based on my personal
knowledge and authentic documents or records;

4. The PETITION is not filed to harass, cause unnecessary delay, or needlessly

increase the cost of litigation; and

5. The factual allegations therein have evidentiary support or, if specifically so

identified, will likewise have evidentiary support after a reasonable opportunity for
discovery;

6. I hereby certify that I have not commenced nor instituted any other action or
proceeding involving the same issue in the instant case before the Supreme Court,
Court of Appeals or any tribunal or agency and that to the best of my own knowledge,
no such action or proceeding is pending thereat; and

7. If I thereafter learn that a similar action or proceeding has been filed or is

pending before the Supreme Court, Court of Appeals or any tribunal or agency, I
undertake to report such fact within five (5) days therefrom to the court or agency where
this petition and certification are filed.
