LETTER OF ACADEMIC CONCERN

Dear Sir/Maam:

academic performance thus far has been UNSATISFACTORY. He / She has been missing out on his/her subject
requirements and has failed to submit learning outputs expected of him/her.

It is essential that he/she become more responsible and compliant with all the academic requirements.
Continued poor performance may result in failure in the subjects, thus the need to take the failed subjects again
in the next school year.
