PROCEDURE
RUMPLE LEED TEST
CODE: SOPIV20 EDITION: 1 PAGE 1/2 1. Definition The Rumple Leed test is an examination of the field of hematology by holding the upper arm for 10 minutes for a diagnostic test of vascular fragility and platelet function. - The Rumple Leed test is an examination of the permeability of the blood vessel walls which is characterized by the appearance of petechiae. - The Rumple Leede (RL) test, also known as the Containment Experiment. The tourniquet test is one of the tests carried out in the field of hematology. This procedure is taught to students so that they understand that this RL test can be used to test capillary resistance and platelet function so that it is a diagnostic effort to detect abnormalities in the primary hemostasis process. At the same time so that students can prepare, implement and interpret the results of this examination. 2. Goal 1. To detect bleeding under the skin (petechiae) as a sign of dengue fever 2. To determine the resilience / fragility of blood vessel walls and the number and function of platelets. 3. Equipments 1. Tool preparation: sphygmomanometer, stethoscope, tray, mat, marker or pen, book, watch 2. Client preparation: - Say hello. - Build a trusting relationship between the nurse and the client. - Clients are notified of the intent, purpose and steps of foot status checks. - Make inspection time contracts with clients. - Adjust the position of the client's feet by straightening the client's legs on the bed. 3. Environmental Preparation: - Maintain client privacy by installing a sampiran or closing the room divider curtain. - Adjust the lighting of the room. - Create a quiet and comfortable environment. 4. Assessment 1. Negative results if in a circle with a diameter of 5 cm, approximately 4 cm distal to the cubital fossa, there are less than 10 petechiae 2. Positive results if in a circle with a diameter of 5 cm approximately 4 cm distal to the cubital fossa there are less than 10 petechiae 3. If during the period of bleeding using the Ivy method, petechiae have occurred, then the results of the dam test (rumple leed) are also positive. 5. Implementation 1. Bring the equipment near the client. 2. Do hand washing 3. Explain the procedure to be carried out 4. Adjust the position in a lying position 5. Take blood pressure measurements 6. Calculate the limit of systolic pressure and diastolic pressure then add up the limits of the two pressures and divide by the formula. FOLDER: 7. Take the MAP measurement by maintaining the pressure of the MAP measurement result for approximately 5 minutes. 8. After that lower the pressure slowly 9. Read positive results - : No petechiae found (<20) + : presence of petechiae (> 20) 10. Record the measurement results 11. Clean up the tools that have been used 12. Tidy back the client. 13. Say hello. 14. Take off the gloves, then throw them in the crook 15. Do hand washing. 16. Document all data collection results in the prepared format. 6. Evaluation Inform the client of the results, and record the measurement results, as well as abnormal findings during the examination. Evaluation result : The client is comfortable after the rumplee leed examination (no complaints) Client privacy is maintained The tools are reorganized 7. Documentation Document the rumple leed test include the patient's name, age, time, location, number of petechiae and nurse initials.
