JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, REYNALDO PERLAS and CARMELITA BERTUMEN, both of legal age, Filipino
citizens, married and residents of Purok 9, Barangay Cogon, Tagbilaran City, Bohol, after being duly
sworn in accordance with law, hereby depose and state that:

1. We personally know FELICISIMO B. BANGLOS, the deceased father of NEMISIO

INGKING BANGLOS, who is a SOCIAL SECURITY SYSTEM Member, being close
family friends since our childhood and our long time neighbor;

2. This affidavit is executed to attest to the truthfulness of FELICISIMO B. BANGLOS’ true

identity;

3. We know for a fact and can vouch that the person of “ZOSIMO BANGLOS” as found on
the SSS records of Nemisio Ingking Banglos as his father’s name, and “FELICISIMO B.
BANGLOS” appearing on the Certificate of Live Birth of Nemisio I. Banglos, issued by the
Office of the City Civil Registrar, City of Tagbilaran, as the Name of the Father of the latter,
refer to ONE and THE SAME person;

4. We further attest that the foregoing discrepancies are due to the inadvertence and oversight
of the Officers in charge of the different agencies leading to various typographical error;

5. We are not related by affinity or consanguinity to FELICISIMO B. BANGLOS and

NEMISIO I. BANGLOS; and

6. This affidavit is executed to attest to the truth of the foregoing facts, inform the proper agency
and for all legal intents and purposes it may serve.
