JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

             WE, NORMAN D. DELOS REYES and IVAN A. JISON, of legal age, Filipino, and with
residence and postal address at Block 09 Lot 06, Amore Subd., Mulawin, Tanza, Cavite and Phase2
Section1, Block 12 Lot 5, Belvedere Subd., Brgy. Paradahan I, Tanza, Cavite likewise of legal age,
Filipino, after having been duly sworn to in accordance with the law, do hereby depose and state:

1. That we personally know JOSE EDWIN B. PATRICIO of legal age, Filipino and with residence
and postal address at Block 24 Lot 40, Walnut Street, Grand Chestnut Grove, Pulong Sta. Cruz, Sta.
Rosa, Laguna.  He is our friends and has known him since childhood;

2. That unfortunately, when he enrolled for primary education, his name was erroneously entered as
EDWIN PATRICIO.  He has been using this name ever since and this appears in all of his education
and employment records as well as in all his other documents;

3. That we hereby state under oath that the names JOSE EDWIN B. PATRICIO, as appearing in his
birth certificate and passport and EDWIN B. PATRICIO as appearing in  all his other records, refer
to one and the same person;

4.  That we are executing this affidavit to support his request to have all his records amended to reflect
the name JOSE EDWIN B. PATRICIO, the name appearing in his birth certificate and passport.

5.  That I am executing this affidavit to attest to the truth of the foregoing facts and for whatever legal
intents and purposes it may deem serve.
