1. Trained/Authorized employees conducting work?

2. Employee undergone not scared of heights, filled in psychological questionnaire

3. Employee not under the influence of alcohol or drugs?

4. PPE: Overalls, gloves, safety footwear, safety harness?

5. Scaffolding platforms clean and clear of debris, unnecessary equipment and material?

6. Is harness being used? Fitted correctly and hooked onto fixed object?

7. Area directly below scaffold clear of other workers and material?

8. Any evidence of employees making alterations to scaffolding without permission?

9. Correct signage (Safe/Unsafe)?

10. Employees using access provided?

11. Weather conditions suitable for working on scaffolding?

12. Workers not overloading scaffold platforms?

13. Any material/equipment stacked close to edge on platforms?
