PTO - Erection of Access Scaffolding

1. Trained/Authorized employees conducting work?


2. Employee undergone not scared of heights, filled in psychological questionnaire

3. Employee not under the influence of alcohol or drugs?

4. PPE: Overalls, gloves, safety footwear, safety harness?

5. Positing of scaffolding suitable and safe, ground suitable for support, allows free access?

6. All material lowered and not thrown to lower level?

7. Is harness being used? Fitted correctly and hooked onto fixed object?

8. Scaffolding material in good condition – no corrosion, bent, worn etc?

9. Base of scaffold square, plumb, level and correct sole boards?

10. Correct sequence of erecting followed – standards, ledgers and then boards?

11. Adequate handrails and toe boards placed on completed platforms?

12. Correct safe/unsafe signage erected at stage of erection and area demarcated?

13. Is the scaffolding tied or supported where required during erection and braced?

14. Weather conditions suitable for working on scaffolding?

15. Scaffold supervisor coordinating erection?

16. Any unauthorized employees on scaffolding during procedure?

17. Employees aware of the fall rescue procedure?
