    PTO - Erection/Dismantling of Formwork Shutters

    CHECKPOINTS CHECK

    1. Formwork supervisor appointed?


    2. PPE for workers: hard hat, overalls, leather gloves, safety boots, harnesses (when required)?

    3. Solid foundation for shutters?

    4. Shutters erected in accordance with design/sketch etc?

    5. Working at heights not to be a problem (Vertigo) – medical fitness or questionnaire completed?

    6. Props plumb, aligned and spaced correctly?

    7. Suitable signage displayed when erection taking place – exposure limited to other workers?

    8. Any overloading on decks etc?

    9. Formwork signed off as safe?

    10. Stripping authorized and required signage in place – area cordoned off?

    11. Safe stripping techniques being used?

    12. All material safety stacked etc?
