CHECKPOINTS CHECK

1. Trained/Authorized employees conducting work?

2. Employee undergone not scared of heights, filled in psychological questionnaire

3. Employee not under the influence of alcohol or drugs?

4. PPE: Overalls, gloves, safety footwear, safety harness?

5. Positing of scaffolding suitable and safe, allows free access?

6. Tower braced and supported where necessary?

7. Is harness being used? Fitted correctly and hooked onto fixed object?

8. Scaffolding material in good condition – no corrosion, bent, worn etc?

9. Base of scaffold square, plumb, level ?

10. Adequate handrails, platform fully boarded and toe boards placed on completed platforms?

11. Correct safe/unsafe signage erected at stage of erection and area demarcated?

12. Correct height to base ratio (3 times base) and wheels locked?

13. Weather conditions suitable for working on scaffolding?

14. If being moved no employees on platforms, guide roped used where possible, height ratio to floor
clearance ie 3m high must have 3m clearance on ground level should it topple?
15. Any unauthorized employees on scaffolding during procedure?

16. Employees aware of the fall rescue procedure?
