After receiving your email about the follow-up inspection performed by Mr. Gates with Hollingsworth Pack last week, I sent emails to both GL Hunt and Hollingsworth Pack requesting for any reports or notes from
that inspection. I received an email from Hollingsworth Pack from March 11, 2020 in regards to a March 10, 2020 inspection. The email indicates that the only area inspected was one corner of the structure. Here is
the email from March 11th:

I would like to schedule a time for DSD inspectors, to include Greg Lehman, Eloy Resendez, and myself to perform an inspection of the structure on Friday May 7th to begin creating a list of items to correct. If the
issue detailed in Mr. Gates’ email has not been corrected, then we will add that to the list as well. Would Friday morning at 10:30 AM work for you?

It just occurred to me that Hollingsworth Pack does not have to send anyone out to the property again.
As I've mentioned several times, their certified engineer Levi Gates has already performed an onsite, visual POST WORK inspection of 1006 Wyoming. He elaborated extensively in my presence on
what needed to be done to correct the errors. Instead, Hollingsworth Pack allowed an unlicensed associate to sign the approval letter submitted to the City. That suggests to me that Gates values his
hard-earned engineering license and wouldn't sign that approval letter.
At any rate, the needed onsite post work inspection has already been performed by a licensed engineer at Hollingsworth Pack.
They can just provide that report.

DSD has reached out to the Engineer of Record for the design of the foundation repair, Mr. Christopher Hewitt with Hollingsworth Pack, to discuss the option of having them complete a list of items to correct
for a code compliant foundation. Mr. Hewitt stated that his company’s HR Department would not allow anyone from Hollingsworth Pack to revisit the property.
We have notified GL Hunt of his response and provided options for developing the list of corrections, to include:

1) If the engineer on record, Chris Hewitt, and his company are not able to perform a site visit, is there another means of providing status of the foundation to him via pictures or remote video. If
so, would he be able to create a list of items to correct through these means? DSD can be present for any efforts to provide this documentation.

2) The engineer on record, Chris Hewitt, does not have to be the individual to produce the list of corrections for review. If GL Hunt is able and/or willing to contract with another structural
engineering firm, then DSD would accept the recommendations form this organization as well. As before, DSD can be present for any onsite inspection performed.

3) If GL Hunt has any other recommendations or suggestions, DSD is willing to vet the options and discuss with the property owner to help find a resolution.

This was submitted to GL Hunt on late Friday afternoon, April 23rd, and we are working with the company to determine another option. Once we have worked out an option with GL Hunt, I will provide the
details to you for approval. Please let me know if there is any additional information I can provide at this time.
