High cost Lower cost + enables water reuse
Removal of micropollutants at the source:
Possibility to remove 100% high concentractions at discharge points
of the micropollutants discharged to water bodies Max. removal of 20%
of all micropollutants discharged to water bodies

“The ministry of environment has initiated a plan since May 2009 with the
aim to pay attention to hospital wastewater”
Karen Ellemann
“Pharmaceutical by-products can be harmful for the environment. Therefore,
it is important that the different administrations take this into account when
authorizing discharge permits to the hospitals.”
“In the last year we have made important progresses in the field of emergent
micropollutants and in the possible technologies to remove them”
“I hope that the administrations and the different regions will initiate a
constructive dialogue in order to find a solution for each and everyone of the
hospital to avoid that these substances reach the environment”

Findings
• MBR itself reduces the concentration of a large number of APIs
analyzed. MBR + polishing = BAT for treatment of hospital
wastewater in DK
• The polishing step in first place (GAC / O3) removes the most
• GAC filters not saturated during the first year of operation
• NO detection of Adsorbable organic halogens (AOX) in the
efluent
• O3 recomended dosage to ensure total API removal: 4 mg O3 / mg
COD
• POZ2 (O3 --> GAC --> UV) is more effective: Removes a larger
number of compounds and prolongs the life of GAC

Toxic and hard-biodegradable antibiotics Factor 10-160 exceeding of effect limits No exceeding of effect limits for
and pain killers for water living organisms water living organisms
X-ray contrast media (eg iomeprol) High concentration (2,5-7 mg/l) 99% removal
Antibiotic resistant bacteria High presence of multiresistant bacteria No faecal or multiresistant bacteria
Ecotox effect - Fish fry (zebra fish) 100 % mortality (death) within 96 hours 0 % mortality (death) within 96 hours
Ecotox effect - Crustaceans (daphnies) No offspring (all daphnies died) Off springs survives like in clean water
Water born viruses High concentration (1.7·105) High concentration (1.7·105)
Estrogenic activity (A-YES) Estrogen effects No estrogen effects
• Decentralized solutions require of robust, efficient, reliable and fully automatic plants
• Still, Danish autorithies have taken action, leading to the first full-scale WWTP installed in a
hospital with the aim to remove pharmaceuticals and antibiotic resistant bacteria
• All pharmaceuticals showed concentractions in the effluent below PNEC for the configuration
O3=>GAC=>UV
• Non ecotoxicologic effects, viruses, antibiotic resistant bacteria, or estrogenic activity were
detected in the effluent
• The effluent quality is similar to drinking water, enabling possibilities for water reuse
• The overall cost is around 1,5 €/m3 representing 64% savings and a payback time around 9 years
