BIOMETRIC GUIDELINES

All regular and casual employees are mandated to strictly comply with the following guidelines on the submission of attachments for
monthly DTR print-out:

1. All offices are mandated to observe on time submission of complete attachments of Regular and Casual Employees on or before the 5 th
day of the month. In case deadline falls on weekends or holidays, it shall be adjusted on the first working day of the subsequent week.

2. Attachments must be arranged in alphabetical order regardless of position/rank.

3. Upon submission to the Human Resource Management Office, complete attachments are considered checked and verified by the
concerned employee/s.

4. HRMO personnel shall review the attachments before printing DTR, if found to be incomplete and/or disarranged, attachments shall be
returned to the respective/concerned office.

5. All UNDER TIME, TARDINESS, and ABSENCES reflected on the DTR PRINT-OUT without attachment/supporting document shall be
deducted in the Leave Credits or Salary, whichever is applicable. Submission of late attachments after the DTR PRINT-OUT to reconcile
UNDERTIME, TARDINESS, and ABSENCES shall NOT be entertained.

6. Non-submission by an employee of Attachment on the specified period will be made as basis for such employee to be dropped from the
payroll until proper compliance be made.

c. Locator slip or Office order For Official Business
