FACTS OF THE CASE:

That on or about 2400H 05 February 2019, Mr. Mario Torres (“Mr. Torres”) was
walking inside Zarate Village, located at Tacay Road, Barangay Pinsao Proper, Baguio City
when Mr. Juan Miguel and Mr. Jun Miguel while drinking at the side of the street in front of a
sari-sari store, threw bottles of beer and hit Mr. Torres at the back of his left leg causing
superficial wound.

After the incident the offenders ran towards their house located at 124 Stone
Crusher, Barangay Pinsao, Baguio City.

DISPOSITION : For record as requested.

This certification is being issued upon request of Mr. Mario Torres in connection with
his filing of a Barangay Complaint.
