ONE HEALTH PASS & PHILHEALTH
 Three (3) days prior travelling, register on
the One Health Pass (OHP) thru
https://www.onehealthpass.com.ph/e-
HDC/ and accurately provide all
required information.
 Ensure to save a copy of the unique
transaction number and QR code which
will be required upon check-in.
 Updated Philhealth membership and
may visit
https://www.philhealth.gov.ph/services
portal to check Member Data Record
(MDR)
AIRPORT ARRIVAL
 Temperature check, present the BOQ
One Health Pass QR Code and your
proof of vaccination to the dedicated
Bureau of Quarantine (BOQ)
representative for re-verification at the
Department of Transportation One-Stop-
Shop (OSS).
 For fully vaccinated passengers, you
may proceed to immigration for your
clearance.
For unvaccinated and partially
 vaccinated passengers, quarantine is
required.
 Clear with immigration and submit the
Arrival Card.
 Claim your baggage at the designated
carousel and clear with Customs Check.
Submit the Bureau of Customs Form.
QUARANTINE / ISOLATION
FULLY VACCINATED NATIONALS
 Negative RT-PCR test result taken within
48 hours prior to departure from the
country of origin
 No facility-based quarantine required
 Self-monitor for any sign or symptom until
the 7th day from arrival
The One Health Pass is an online
platform for people visiting the
Philippines. It was introduced to
facilitate safe travel to the Philippines
during COVID-19.
Incoming passengers need toregister
their details and flight information
before taking a flight to the
Philippines.
Through the One Health Pass,
travelers complete the mandatory
electronic Health Declaration Card.
Regenerate your VaxCertPH if you
already have it, or get your
VaxCertPH 48 hours after your full
vaccination.
Visit https://vaxcert.doh.gov.ph or
drop by your LGU to generate your
own VaxCertPH.
QUARANTINE / ISOLATION
UNVACCINATED, PARTIALLY
VACCINATED, UNVERIFIED NATIONALS
 Negative RT-PCR test result taken
within 48 hours prior to departure
from the country of origin
 Facility-based quarantine until the
release of the negative results taken
on 5th day from arrival.
 Self-monitor for any sign or symptom
until the 14th day from arrival
WHILE ON VACATION
 Complete Exit Interview in Crew Self
Service (CSS) portal
 Review document checklist and
principal required documents
 Update information and upload
crew documents in CSS portal for
Fleets ‘verification
 File in advance documents for
renewal to avoid delays for
processing with atleast 15 months’
validity of crew documents
VESSEL ASSIGNMENT
 Fleet shall notify crew for possible
vessel assignment and reporting
protocols
 Follow the guidelines for submission of
documents thru courier and online
transactions
 Reporting crew shall observe14-day
home quarantine prior going office
 Approved appointment through
Crew Self Service or via email
 Present rapid test taken at any
medical clinic or available in
Barangay issued at least 7-days prior
your scheduled appointment
