PH Bureau of Immigration Declaration Form and Health Declaration Certificate (Health Code)
NOTE: The following documents must also be uploaded in the Health Declaration Certificate/Health Code Application:
1. Negative Nucleic Acid Test result valid within forty-eight (48) hours prior departure
2. Valid passport's biodata page or any travel document
3. Data page of the valid Chinese Visa for Non-Chinese National (not required for those eligible for visa exemption)

Passengers from the Philippines to China, effective 1st of March 2023, are requested to make health declaration with a
negative COVID-19 test result either through nucleic acid test or self antigen test within 48 hours before boarding.

A Health Declaration Form can be obtained through Wechat mini program of China Customs (QR code) or Health
Declaration official website.

NOTE: Airlines no longer review pre-flight nucleic acid test and antigen test results for passenger going to China, the airlines
just check passengers' QR code for the Custom declaration.
