To the participant,
Greetings! This is an invitation to participate in a study entitled "The Meaning
of Virtual Return Demonstrations to a Nursing Student of Capitol University:
A Phenomenological Inquiry ". The purpose of this study is to understand the
experience on having an virtual return demonstration.
This study is to be conducted by Mr. Kent Colanse, Ms. Jennifer Lorenzana and
Mr. Derek Sarte, who are 3rd-year nursing students at Capitol University.

To participate in this study, the following guidelines apply:

 The researchers will conduct an online survey with you that you can answer
during your free time. The researchers ensures the privacy and protect the
identity of those who answered the survey questionnaires.
 The researchers will give you a copy of the questionnaire to be answered.
 Some basic profiling information may be obtained. However, your identity will
NOT be included in the output of this study.
 Your participation to this study is VOLUNTARY, with NO monetary
compensation or its equivalent.
 You have the right to withdraw at any time, without prejudice.
 You are entitled to full disclosures, and to ask questions throughout the
duration of your participation.
