ACADEMIC APPEAL REQUEST FORM

The PUP Student Handbook particularly in Section 12 provides the University’s
policy on Scholastic Delinquency, viz.:

12.1 Each college shall implement the following rules on scholastic delinquency:

12.1.1 Warning. Any student who at the end of the semester

obtains final grades of “5” in 15% or less of the total number of
academic units in which he/she is registered shall be warned by the
Dean or Director concerned to improve his/her work; if he/she fails or
gets Incomplete marks in 16-30% of the total number of academic
units in which he/she is registered, he/she shall be warned by the
Dean and his/her load shall be reduced by 3 units. (underscoring

12.1.2 Probation. Any student who at the end of the semester

obtains final grades of “5” in 31-50% of the total number of academic
units in which he/she has enrolled shall be placed on probation for
the succeeding semester and his/her load correspondingly reduced
by 6 units by the Dean or Director concerned. (underscoring
supplied)

12.1.3 Dismissal. Any student who, at the end of the semester,

obtains final grades of “5” in 51-75% of the total number of academic
units in which he/she receives final grades shall be dropped from the
rolls of the College concerned. If more than 75% he/she shall be
dismissed from the University.

Subjects with ‘WITHDRAWN” mark as well as “OFFICIALLY

DROPPED” subjects should not be included in the computation of
the General Weighted Average. (Title 5, Section 8.4, Academic
Regulations Procedure and Services)

Students who are subject to DISMISSAL based on the above rules

may opt to file an appeal to the Office of the Dean. It is understood
that the grant of the appeal is merely discretionary on the part of the
Dean and not a matter of right of the student concerned.
Furthermore, the grant of the appeal may be subject to other terms
and conditions, as may be necessary, including but not limited to the
limitation of units that the student may enroll in.

The appeal procedure may be revoked upon the sound discretion of

the Dean.
