Achievement Test ❖ Review the students on scheduled time given by the
school.
❖ Practiced them how to fill-up the NAT form as well as
how to shade it.
❖ Give them final review test in preparation for the
actual NAT

level of students to serve as guide for principals and teachers
in their respective courses of action.

Modular Distance Assessment Activities:


Learning ❖ Performance task, series of practice tests or send
reviewer to be guided by the parents or facilitator ❖ The
teacher takes the responsibility of monitoring the
progress of the learners. The learners may ask
assistance from the teacher via e-mail, telephone, text
message/instant messaging among others. Where
possible, the teacher shall do home visits to

learners needing remediation or assistance.


Printed Modules will be delivered to students,
parents or guardians by the teachers or through
the Local Government Officials.

Action/Decision:

Action/Decision:
Expansion Phase of Face to Face Classes with Caption
❖ Comply the SSAT in order to commence the Face to
face classes
❖ Conduct Learners Survey of vaccinated and
unvaccinated students
❖ All public schools nationwide start conducting self-
assessment using the School Safety Assessment
Tool in preparation for the expansion phase of the
DOCUMENTATION face-to-face classes.
